项目摘要

本研究の目的は、終末期患者家族が保有するPost-intensive care syndrome-family(PICS-F)の危険因子の特定を可能とさせるリスクアセスメント方法を明らかにし、臨床実践可能なリスクアセスメントツールを開発することである。初年度は、PICS-Fリスクアセスメントツール開発の基盤となるPICS-F危険因子を既存のエビデンスから抽出・特定するためのシステマティックレビューおよびメタ解析を実施した。システマティックレビューおよびメタ解析の結果からは、「女性」「精神疾患の既往歴」「高い患者の重症度」「患者の配偶者」等の患者・家族の社会人口統計学的特徴に起因した危険因子が特定された。また、「ICUスタッフと家族とのコミュニケーション不足」が家族のICUでの体験に起因した修正可能な危険因子であることが特定された。さらに、ICUスタッフと家族とのコミュニケーションに関するどのような要素が危険因子となっているのか、その詳細は明らかにされておらずリサーチギャップとなっていることが分かり、本研究において明らかにする必要があると判断された。そのため、PICS-F危険因子の詳細を特定する国内での追加調査の実施に向けた研究実施体制整備とプロトコル作成を実施、次年度での調査実施に向けた体制整備を完了した。なお、初年度に実施したシステマティックレビューおよびメタ解析の結果については、国際誌への論文投稿とし、既に受理され公表されている。

The purpose of this study is to clarify the possibility of specific risk factors for the Post-intensive care syndrome-family(PICS-F) that can be retained in the family of end-stage patients, to clarify the access management approach, and to develop the access management approach in clinical practice. In the first year, the PICS-F risk factor was extracted from the existing database and analyzed. The results of the analysis include: "female,""history of mental disorders,""severity of high patients,""spouse of patients," and other sociodemographic characteristics of patients and families. "ICU family health problems" is the cause of ICU family health problems. In addition, the ICU staff and family members are required to determine the risk factors and details of the study. The implementation direction of the additional investigation in China and the preparation of the investigation implementation direction of the next year are completed. In the first year of the year, the results of the analysis were submitted to the International Journal and accepted.
