项目摘要

新型コロナウイルス感染症流行の影響で、必要な打ち合わせが8月まで行えなかった。9月以降、研究分担者及び他施設の研究協力者とアプリ開発業者との会議を2-3週に1回程度実施し、アプリの仕様を検討した。当初は、鑑別疾患を上げるところまでを検討していたが、鑑別疾患を考える上では思考のプロセスを医師と入力者で議論することが必須であるため、まず重症度の判定のみのアプリ開発を行う事とした。現在稼動している新生児の状態入力システムでは、鑑別疾患を入力者にも提示出来るものになっている。バックヤードでは入力した情報から重症度が判定され保持される機構となっているため、今回はこの機構を利用することとした。1.新生児の状態の提示：新生児の状態については、(1)看護師が自分で状態を選択する、(2)アプリ内でランダムに状態を提示する　の2つの方法で提示出来るようにした。2.重症度レベル：1問題なし、2乳児室で観察可能、3GCUレベル、4NICUレベル、5致死的、の5段階とした。これは、現在稼動しているアプリで使用している重症度をそのまま利用したものである。3.重症度の選択：看護師は、1.で入力または提示された新生児の状態が、上記の重症度レベルのどれにあたるのかを選択する。4.正誤判定：アプリは看護師が入力したものの正誤を表示する。個人のスキルアップの検証の他、経験年数などの違いによる正答率の違いについても検証可能な形とするため、事前の登録で、看護師にはIDを付与するが、その際、年齢、性別、看護師としての経験年数、NICUでの経験年数、特定看護師などの資格の有無の情報を取得する。
