本研究は壮年期を対象とした家庭血圧管理事業である益田研究(Masuda study)を調査フィールドとし，年単位の伴走的な血圧管理事業が参加者に与える心理的・社会的・医療経済的な影響を評価することである。本研究課題を遂行するために，①社会的孤立と血圧管理状況との関連，②伴走的支援の「場」で生じる現象と社団の役割との検討，③血圧管理状況（測定状況）を要因とした年間医療支出額の分析，④益田研究参加前後における参加者の主観的健康観の変容，に関する調査を実施する。初年度である本年度は，調査フィールドにおける研究実施体制を構築した。さらに益田研究参加者を対象とした質問紙調査を実施した。質問紙調査は，家庭血圧値に関連する生活習慣だけでなく，参加者の生活環境や心理状況に関する質問項目が設定された。就労状況や家族構成，社会的孤立や主観的幸福感等を含む質問紙調査を実施した。今後具体的な分析を進め，随時，研究成果として報告していく予定である。また，研究成果の報告として，申請者及び分担研究者の所属する各学会における積極的な研究成果報告を実施した。一例として，参加者の高血圧の認知率や管理率は，本事業が継続されるに従い，上昇していることがわかった。特に認知率と受診率は上昇傾向にあった。本事業における寄り添い形の支援が，疾病の重症化を予防し，早期の医療受診を促している結果の表れである。ただし，管理率は伸び悩む傾向にあった。さらに，高血圧と社会的要因に関しては，就労状況が高血圧に関連していることがわかった。ただしこの傾向は性差が認められた。男女で就労環境に差があっただけでなく，雇用状態や職種によって高血圧者の割合が異なることがわかった。

This study investigated the psychological, social, and medical effects of a family's blood pressure management program on participants in their prime. This study was conducted on the following topics: (1) social isolation and blood pressure management status and correlation;(2) analysis of associated support "field" phenomenon and social service division;(3) analysis of blood pressure management status (measurement status) and medical expenditure during the period;(4) health status of participants before and after participation in Yitian study; and (5) implementation of related surveys. In the beginning of this year, the research and implementation system was established. The participants in the study were questioned by the paper survey. The questionnaire includes questions about the lifestyle of family blood pressure, the living environment and psychological status of participants. The paper survey was conducted with questions about family status, family composition, social isolation, and the happiness of the host. In the future, specific analysis will be made, and the research results will be reported at any time. In addition, the applicant and the participating researchers should actively report their research results. For example, the participants 'high blood pressure recognition rate and management rate will increase in the cause. The rate of recognition tends to increase. We provide support to prevent disease from becoming more severe, and promote early medical treatment. The management of the company Today, high blood pressure and social factors are related to each other, and high blood pressure is related to each other. The difference between the two sexes is obvious. Men and women are different in terms of their environment, employment status, and high blood pressure.
