2022年度の主な実績は、先行調査の介護施設における看護師と介護士への認知症高齢者に対する排便ケアの教育プログラムの有効性に関する実証研究を米国消化器病学会誌「The American Journal of Gastroenterology」にて公表したことである。研究成果の概要は、次のとおりである。慢性便秘は、介護施設に入所している認知症高齢者によくみられ、興奮・徘徊・暴言など認知症の行動・心理症状（BPSD）を引き起こし、介護者に多大な負担を与える可能性がある。私たちの研究グループは、介護施設に入所する認知症高齢者を対象に、排便習慣の確立（Bowel training）と排便を促進する効果的な排便姿勢を組み合わせた排便ケア（介入群）と水分・食事摂取の促しや下剤管理など一般的なケア（対照群）とを比較することで、慢性便秘と関連する問題を改善するか検証した。研究成果のポイントは次の3点である。①認知症高齢者を対象に、排便習慣の確立（適切な排便時間と間隔でトイレ誘導）と排便を促進する効果的な排便姿勢（足底を床につけた前傾姿勢）により慢性便秘の改善に有効であったことを世界で初めて実証した。②残便感のない自発的排便（24時間以内に下剤や浣腸など治療を受けていない排便）が、8週間の排便ケア後に有意に改善した。③認知症高齢者の生活の質および精神状態が改善し、さらに介護者の負担感も軽減することを明らかにした。さらにこの成果は、メディア報道（新聞）を通じて幅広い読者を対象に公表された。その他、英国科学雑誌への掲載が決定している。この掲載の目的は、本研究課題の取り組みに関する広報活動および、研究内容に賛同してもらい共同研究の可能性などを探索していくことである。

The 2022 annual results were published in The American Journal of Gastroenterology, an empirical study on the effectiveness of educational programs for caregivers and nurses with cognitive disabilities. Summary of research results,. Chronic constipation, how much burden and likelihood of intervention are involved in cognitive hyperactivity, excitement, wandering, violent speech, behavioral and psychological symptoms (BPSD). The study of privacy, intervention settings, cognitive disorders, Bowel training, promotion of effective bowel movement posture, combination of bowel movement (intervention group), moisture, food, promotion of lower management, general constipation (contrast group), comparison, improvement of chronic constipation related problems. The results of the study are as follows: 1. For those with high cognitive disorders, the establishment of defecation habits (appropriate defecation time interval and induction), the promotion of defecation, the effective defecation posture (foot bed forward posture), the improvement of chronic constipation, and the beginning of the world. 2. Defecation due to residual stool feeling (defecation within 24 hours after treatment), and intentional improvement after defecation failure within 8 weeks (3) The quality of life and mental state of patients with cognitive disorders are improved, and the burden of caregivers is reduced. The results of this report are as follows: He and the British Journal of Science published the decision.
