本研究の目的は「簡易型CBT-Iを妊娠期の夫婦を対象に実施し、産後うつ病のリスク改善効果をRCTにより検証する」ことである。その初年度にあたる2022年度の計画は、既存のCBT-Iをもとに、妊婦の生理的変化や生活様式を考慮して簡易型CBT-Iプログラムを作成することであった。プログラムの作成にあたって、妊婦の睡眠生理や不眠に関する最新の知見を得るために、妊婦を対象としたCBT-Iの内容とその効果について研究論文（総説、原著）をレビューした。また、最近では関連学会からの妊婦の不眠に関するガイドライン策定や、睡眠指針の改定の動きなどがあり、それらの報告書なども集めプログラム作成に有用な情報を収集した。妊婦の生活様式などについては、妊婦の生活様式を良く知る助産師および保健師にインタビューを実施し、その実態を踏まえたうえでプログラムの内容を検討した。これらの結果から、介入の強度（組み込むCBT-Iのポイントと回数・時間など）、妊娠期の夫婦の生活様式を考慮した教育内容（妊娠による睡眠の変化、出産前後の夫婦の睡眠状態の変化、新生児の生活リズムと泣きの理解など）の大筋を決定した。また介入を実施するにあたり、オンラインコンテンツの配信方法や効果評価のためのデータ収集の方法などを検討し、技術的な準備を進めた。実際に介入を実施するためには、2022年度に決定したプログラム内容や介入実施方法の概略に関してさらに関係者や当事者の意見を反映させる必要があるため、2023年12月までを目途に再検討し具体的なコンテンツの作成を目指したい。

The objective of this study was to investigate the effects of CBT-I on pregnant couples and postpartum patients. In the first year of 2022, the plan is to consider the existing CBT-I and the physiological changes of pregnant women, and to create a simple CBT-I platform. The latest knowledge on the development of sleep physiology and insomnia in pregnant women has been obtained. The content and results of CBT-I in pregnant women have been studied. Recently, the Institute has published a report on how to make useful information about the sleep status of pregnant women. The life style of pregnant women is very important. The midwives and health care professionals are familiar with the life style of pregnant women. The contents of the life style are discussed. The results of the study include: the intensity of intervention (group of CBT-I, number of cycles, time); the life style of pregnant couples; the educational content (sleep change during pregnancy, sleep state change of couples before and after delivery, understanding of newborn life). The method of information distribution and the method of evaluation and collection of information are discussed and prepared. In 2022, we decided on the content of the intervention and the outline of the intervention implementation method, and we discussed the specific content of the intervention in December 2023.
