本研究の目的は高齢RA患者の運動アドヒアランスを高めるアクティブゴール設定アプローチの構築である．研究の第一段階として，今年度の目標は医療提供者と患者にインタビュー調査を行い，運動のアドヒアランスを高めるために必要なゴール設定の実例と質問を抽出することであった．研究内容：2022年度は関節リウマチの手に有用なハンドエクササイズプログラムSARAHの運動処方を実施した．実施人数は2名であった．対話のセッションでのインタビュー聴取と，1名に対する超音波画像を用いた炎症の評価を加えて実施状況におけるリスク対策を実施した．ゴールセッティングについては，初回設定時は身の回り動作・家事といった自宅内での手を使う動作での設定で複数の提案が対象者より得られた．また，インタビューを通して生活課題に関する困りごとやニーズに関する情報を得た．これらを活用し，ゴール設定に関するセラピスト側の質問例を収集している．今後の参加者募集拡大に向けて，日本リウマチ友の会の会長に研究協力依頼を行った．結果：運動処方については，介入期間（12週間）の実施回数は週3回以上であった．自主トレ期間は現在調査中である．なお，これまでの成果については，第31回日本リウマチ学会近畿支部学術集会で報告（口述演題）を行った．今後：現在調査中のデータの解析と，日本リウマチ友の会を介した募集に関する実施方法の検討を行う．また，本研究で用いるSARAHは2023年より患者教育サイトを新たに公開する．その実施方法と運営方法について原著者らから学ぶことを計画している．
