This subproject is one of many research subprojects utilizing the resources provided by a Center grant funded by NIH/NCRR. The subproject and investigator (PI) may have received primary funding from another NIH source, and thus could be represented in other CRISP entries. The institution listed is for the Center, which is not necessarily the institution for the investigator. This study has 2 components. Phase 1 identifies patients who have progressive chronic kidney disease despite optimal conservative management including the use of adequate blood pressure control and a low protein, amino acid supplemented diet. In phase 2 those patients with a progressive decrease in kidney function despite these measures are treated with 200 mg/day of ketoconazole plus low dose prednisone (2.5 mg/day). Currently two patients with insulin-dependent diabetes mellitus and nephropathy continue to receive ketoconazole plus prednisone and maintain independent renal function. Six patients continue to be followed in phase 1 of the study and continue to show stable renal function. Three additional new patients were entered into Phase 1. One had very advanced kidney failure at the time of initial assessment and rapidly progressed to dialysis. The other two are currently being followed to assess the degree of progression of their kidney failure.
