This subproject is one of many research subprojects utilizing the
resources provided by a Center grant funded by NIH/NCRR. The subproject and
investigator (PI) may have received primary funding from another NIH source,
and thus could be represented in other CRISP entries. The institution listed is
for the Center, which is not necessarily the institution for the investigator.
In our institution, gentamicin dosing for term and near-term neonates changed in April 2000 from a q12h interval to a q24h interval. During the ensuing 15 months, we discovered a higher incidence (3.5%) of hypocalcemia compared to the incidence (0.4%) detected in a retrospective review of cases of all infants admitted to the Observation Nursery within the Newborn Nursery during the 15 months prior to the dosage change when gentamicin as given q12h. Previous studies have demonstrated increased urinary excretion of calcium with gentamicin administration, but have not documented alterations in serum calcium levels. We propose that administration of gentamicin alters not only urinary calcium but also serum calcium. We plan to measure serum and urine electrolytes in three groups of infants: 25 infants receiving q12h gentamicin, 25 infants receiving q24h gentamicin, and 10 control infants not receiving gentamicin.
