This subproject is one of many research subprojects utilizing the resources provided by a Center grant funded by NIH/NCRR. The subproject and investigator (PI) may have received primary funding from another NIH source, and thus could be represented in other CRISP entries. The institution listed is for the Center, which is not necessarily the institution for the investigator. This is a 4-year observational follow-up study of children enrolled in the Childhood Asthma Management Program (CAMP). CAMP was a multicenter randomized clinical trial designed to determine the effects of three treatments (albuterol, albuterol plus inhaled steroids, albuterol with inhaled non steroids) in children age 5-13 years with mild to moderate asthma. The hypothesis of this study is that inhaled corticosteroids used during CAMP will not benefit lung function but will decrease airway responsiveness, will not affect height and bone density, and will reduce persistence, remission and relapse of symptoms with resultant need of medication. Furthermore, increase in lower respiratory symptoms, allergy, blood eosinophilia, airway responsiveness, and personal smoking will decrease lung function. Finally, certain genotypic variants will be associated with impairment of growth of lung function caused by asthma. Three Specific Aims will be examined including 1) the effects of lower respiratory symptoms, allergy, blood eosinophilia, airway responsiveness, and personal smoking on attained level of lung function and reactivity in young adulthood 2) effects of lung function and airway reactivity, allergy, eosinophilia and personal smoking on lower respiratory symptoms in young adulthood and 3) factors associated with low FEV1/FVC in CAMP subjects versus normal subjects. This study will be the first study of childhood asthma to define the determinants of outcomes in young adulthood and the first to examine the effects of intensive treatment with potent anti-inflammatory agents on outcomes.
