INFLAMMATORY GENE POLYMORPHISMS IN CARDIAC SURGERY

Cardiopulmonary bypass (CPB) is a potent trigger of the systemic inflammatory response, which includes activation of leukocytes and production of multiple pro-inflammatory mediators. Its consequences include multi-organ system dysfunction, capillary leak, altered coagulation, and increased morbidity and mortality. In sepsis, a condition in which the inflammatory response shares mechanistic and clinical similarities to that seen with CPB, polymorphisms of specific inflammatory genes have been associated with alterations in inflammatory cytokine production and increased mortality. Based upon this information, we propose a prospective observational study to investigate the relationship between inflammatory gene polymorphisms, production of circulating inflammatory mediators, and clinical outcome for infants undergoing cardiac surgery with CPB.
