This subproject is one of many research subprojects utilizing the resources provided by a Center grant funded by NIH/NCRR. The subproject and investigator (PI) may have received primary funding from another NIH source, and thus could be represented in other CRISP entries. The institution listed is for the Center, which is not necessarily the institution for the investigator. The overall goal of the Clinical Core of The University of Kentucky Alzheimer's Disease Research Center (UK ADRC) is to support the research mission of the ADRC and provide excellent clinical diagnoses and care for patients with Alzheimer's disease (AD) and non-AD dementing disorders. It will: (1) maintain a normal control (BRAiNS clinic) of 500 initially normal aged subjects, who will be followed longitudinally to autopsy. Emphasis will be on normal aging and transition to Mild Cognitive Impairment (MCI) and early AD; (2) maintain a Research Clinic of 200 subjects with probable AD and other non-AD dementias who will be followed longitudinally to autopsy; and (3) establish a group of 100 initially normal rural Appalachian aged subjects followed longitudinally to autopsy in the Berea Rural Appalachian Satellite Clinic for comparison to the urban based BRAiNS Clinic. The control participants are evaluated annually using neuropsychological tests to detect subtle changes in memory and other cognitive function designed to detect transition to MCI and dementia. Many existing and planned research projects at UK are dependent on autopsy specimens from normal subjects and those with MCI or early AD. The core holds important consensus diagnostic conferences and relates closely to the Neuropathology and Biostatistics and Data Management Cores. The Clinical Core will provide expert dementia diagnostic services in a referral Memory Disorders Evaluation Clinic for approximately 300 patients per year, a group which serves as a recruiting pool for the Research Clinic and for clinical trials and other research studies. The core will expand and maintain a clinic for African Americans in Lexington, the Kentucky Clinic North Minority Emphasis Clinic, providing longitudinal care and broadening research opportunities to an understudied population. This core participates in the NIA LOAD genetic initiative, NACC funded studies and ADCS trials. All appropriate data are collected and submitted to NACC in a timely manner.
