This subproject is one of many research subprojects utilizing the resources provided by a Center grant funded by NIH/NCRR. The subproject and investigator (PI) may have received primary funding from another NIH source, and thus could be represented in other CRISP entries. The institution listed is for the Center, which is not necessarily the institution for the investigator. The goal is the application of optimal control techniques to develop and test a program of monitoring specified outcomes (controlled glucose level) on an individual basis for a sample of patients with Type I Diabetes. Time series modeling techniques have been used by engineers to ensure the best possible outcomes in manufacturing processes. Such models can be adapted to the needs of medicine to increase the positive results of interventions, which will be planned for future research studies. Basically, a criterion for determining a good outcome (optimal range of glucose level) is established. Regular measurements are taken to monitor the outcome. If the outcome is moving in an undesired direction, the level or content of the intervention is modified. At the next measurement occasion, the direction of the outcome is checked for improvement. Additional modifications can be made to move the outcome in the desired direction. Unlike many other intervention strategies, the degree or content of the intervention can be adjusted on an individual basis. It is well-known that medications and standard dosages do not work equally well for everyone. This program would inform the practitioner of a need for adjustment or continued maintenance of a particular medicine and dosage. This medication monitoring program will differ from others in that it tailors the treatment to the needs and responses of the individual. It also will be informed by frequent dietary and exercise assessments. Adjustments in recommendations regarding diet and exercise will be made in conjunction with the medication adjustments.
