This subproject is one of many research subprojects utilizing the resources provided by a Center grant funded by NIH/NCRR. We hypothesize that periodontal diseases that fail to respond to conventional therapy will benefit from combined cleanings and antibiotics. Our aim is to determine whether scaling and root planing (SRP), local antibiotic and systemic antibiotic therapies, and weekly professional cleaning, can eliminate or suppress periodontal infections in subjects with refractory periodontal disease. Refractory subjects will be assessed clinically and samples of subgingival plaque taken. These samples will be evaluated for their bacterial species content. After initial monitoring, subjects will receive full-mouth SRP under local anesthesia and the placement of doxycycline gel (Atridox) at teeth with pockets >4 mm. Immediately following the SRP and final doxycycline gel removal, subjects will be put on a regimen of systemically administered metronidazole and systemically administered amoxicillin. Subjects will also begin a professional cleaning regimen, which will consist of a once a week removal of supragingival plaque. Maintenance SRP will be performed every three months for the duration of the study in all subjects. Treatment will also include home care.
