This subproject is one of many research subprojects utilizing the resources provided by a Center grant funded by NIH/NCRR. The subproject and investigator (PI) may have received primary funding from another NIH source, and thus could be represented in other CRISP entries. The institution listed is for the Center, which is not necessarily the institution for the investigator. This research is being done to investigate what factors cause Chronic Kidney Disease (CKD) to become worse in children. CKD is when the kidneys have trouble removing waste products from the blood. The number of people with CKD continues to grow, and about 650,000 Americans will have CKD by the year 2010. Because the increase of CKD has been most dramatic in the adult population, few studies have been performed in children with CKD. However, because of the importance of growth and neurocognitive development in the pediatric age group, children are likely to be much more vulnerable to the effects of progressive CKD than adults. Additionally, as kidney disease is usually due to a primary urologic problem or glomerular disease rather than a secondary process such as diabetes or hypertension (HTN), as in adults, the risk factors and consequences of kidney disease progression, independent of other complicating factors, are likely to be clearer in children. In order to treat the problems caused by CKD in children, it is necessary to understand what causes CKD to get worse. The primary goals of this study are to determine what factors cause CKD to get worse in children, and to see how CKD affects a child s brain development and behavior, heart function, growth and general well-being.'
