Eleven million Canadians with chronic diseases account for an estimated 70% of the total health care cost. The World Health Organization advises that increasing the effectiveness of adherence interventions to reduce risk factors and prevent chronic disease may have a far greater impact on the health of the population than any improvement in specific medical treatments. We will address the following research questions to improve adherence. How can we assess and predict a persons health risk of a chronic disease with data on lifestyle, demographics, culture, diet and exercise, attitude and motivation, health and medication beliefs, in addition to the clinical indicators? What meaningful health behavior clusters can be developed to help professional teams deliver targeted health care communications and personalize coaching? What are the driving factors of lifestyles and behaviors for a successful Chronic Disease Prevention and Management? How can we turn data into strategies and recommendations to transform the high risk people into low risk category?
