原发性痛经(primary dysmenorrhea,PD)是常见危害女性身心健康和生活质量的慢性炎症疾病,前列腺素(prostaglandins, PG) 是其发病的关键因素。近年来,NLRP3炎症小体作为炎症反应的核心介导了多种妇科疾病的发生和发展过程,成为了原发性痛经干预的重要潜在靶点。本课题组前期研究初步证实,电针可通过调控NLRP3炎症小体活化治疗PD,但NLRP3炎症小体在原发性痛经中的发病机制尚不清楚。本研究以NLRP3炎症小体参与PD的机制为切入,围绕“NLRP3炎症小体活化”和“炎症级联反应”两个环节,探讨NLRP3炎症小体通过NLRP3→IL-1β→NF-κB→COX2→PG通路引发原发性痛经的发病机制。并以此为基础,研究电针对该通路的干预作用以及该通路在电针镇痛和抗炎效应中的参与。本项目的开展将揭示原发性痛经发病新机制,并为针刺治疗PD提供新的理论依据。

Primary dysmenorrhea (PD) is a common chronic inflammatory disease that endangers women's physical and mental health and quality of life. , prostaglandins (PG) are considered as the key factor in the pathogenesis of PD. In recent years, NLRP3 inflammasome, as the core of inflammatory response, mediates the occurrence and development of various gynecological diseases, and becomes an important potential target of primary dysmenorrhea intervention. The preliminary study of our group confirmed that electroacupuncture can treat PD by regulating the activation of NLRP3 inflammasome。 However, the pathogenesis of NLRP3 inflammasome in primary dysmenorrhea is not clear. In this study, the mechanism of NLRP3 inflammasome participating in PD was taken as the starting point, and the pathogenesis of primary dysmenorrhea induced by NLRP3 inflammasome through NLRP3 → → IL-1 β → NF - κ B → COX 2 → PG pathway was discussed, focusing on "activation of NLRP3 inflammatory corpuscles" and "inflammatory cascade amplification". On this basis, the intervention of Electroacupuncture on this pathway and its participation in the analgesic and anti-inflammatory effects of electroacupuncture were studied. The development of this project will reveal the new pathogenesis of primary dysmenorrhea and provide a new theoretical basis for acupuncture treatment of PD。
