全球3亿多哮喘患者,疾病负担重/反复发作,逐年发病上升明显。近年GINA显著提升了哮喘控制率,但对于EOS介导以外其他表型多控制不理想。已证实ILCs对哮喘免疫调节效应,尤其ILC2s、ILC3s;2018 science:新发现PNECs通过CGRP促进ILC2s/IL-5/EOS活化,致使哮喘发作/加重。基于前期青年基金,项目组围绕哮喘温化寒痰治则经典方“苓桂术甘汤/阳和汤”对ILCs调控机制研究,发现阳和汤能够明显降低哮喘小鼠IL5、IL17、CGRP表达,降低OVA激发哮喘小鼠发作程度,且临床疗效显著。课题计划:通过系列ILC2s、ILC3s细胞体外实验、Ascl1fl-/-、Rag-/-、calcrl-/-致敏哮喘小鼠多靶点反证实验,阐明阳和汤对PNECs/CGRP/ILC2s/IL-5-EOS等肺内区域免疫活化调控靶点及作用机制,丰富中医温阳化痰辨治哮喘的理论内涵。

More than 300 million people suffer from asthma in the world. Asthma has a heavy social and economic burden and it is relapsed. The incidence of the disease increased significantly year by year. GINA has significantly improved asthma control in recent years,but it is not ideal for other phenotypic to control except eos-mediated. It has been proved that ILCs have immunomodulatory effects on asthma, especially ILC2s and ILC3s. In 2018, SCIENCE found that PNECs promotes the activation of ILC2s/ IL-5 /EOS through CGRP, resulting in asthma attack or exacerbation. Based on the early Youth Fund, the project team focuses on the classic prescription of asthma “warming Yang, cold resistance and expectoration” treatment of Lingguizhugan Decoction/ Yanghe Decoction to regulate ILCs mechanism, we found that Yanghe Decoction can significantly decreased the expression of IL5, IL17 and CGRP in asthmatic mice, the attack degree of asthma induced by OVA was decreased, and the clinical effect was significant. Subject plan: through a series of in vitro experiments on ILC2s, ILC3s cells, Ascl1fl-/-, Rag-/- and calcrl-/- sensitized asthmatic mice, this study aims to clarify the regulation targets and mechanism of Yang he Decoction on PNECs /CGRP/ILC2s/ il-5-eos and other intrapulmonary regional immune activation, and enrich the theoretical connotation of TCM warming Yang resolving phlegm and treating asthma.
