炎性肠病(IBD)引起的内脏疼痛可能与肠道炎症、内脏高敏性、脑-肠-微生物群轴功能紊乱和应激等有关,严重影响患者生活质量。基于脑-肠-微生物群轴理论,功能磁共振成像(fMRI)对引起内脏疼痛的肠道功能性疾病的中枢功能变化研究较多。中枢神经在IBD等肠道器质性疾病的内脏疼痛所起的作用以及其作用机理尚未完全明了。静息态fMRI研究发现IBD的常见类型克罗恩病(CD)腹痛患者存在多个脑区激活,躯体疼痛MRS实验研究揭示大脑神经递质谷氨酸(Glu)与γ-氨基丁酸(GABA)等的作用。我们前期通过MRS和fMRI初步研究发现CD腹痛与非腹痛患者大脑前扣带回等区域的Glu和GABA浓度及局部脑区功能连接存在变化。本项目拟通过高分辨力MRS、静息态fMRI等技术探索脑-肠轴回路中大脑皮质神经递质Glu/GABA和相应脑区功能连接变化对IBD内脏疼痛的介导作用,为临床IBD腹痛患者的诊治提供新的理论依据。

The visceral pain in patients with inflammatory bowel disease (IBD) might be related to intestinal inflammation, visceral hypersensitivity, brain-gut-microbiota axis dysfunction, and stress. It has a serious impact on patient's quality of life. Based on the brain-gut-microbiota axis theory, functional magnetic resonance imaging (fMRI) has been widely applied to study on the changes in cerebral cortex function in gastrointestinal functional diseases causing visceral pain. The role of the central nervous system in visceral pain in gastrointestinal organic diseases such as IBD and its mechanism are poorly understood. Researchers have found that many brain regions are activated in Crohn's disease (CD) patients, a common type of IBD, with abdominal pain. The MR spectroscopy (MRS) experiment of somatic pain revealed the role of brain neurotransmitter glutamate (Glu) and γ-aminobutyric acid (GABA). In our recent study conducted by MRS and fMRI, the alterations of the excitatory neurotransmitter Glu and inhibitory neurotransmitter GABA concentrations and functional connections in the anterior cingulate cortex in patients with CD abdominal pain comparing with non-abdominal pain CD patients were found. Therefore, we would explore the modulating effect of imbalance of Glu/GABA and functional connectivity changes of corresponding brain regions in the brain-gut-microbiota axis circuit on visceral pain of IBD by using high-resolution MRS and resting-state fMRI. This investigation may provide a novel theoretical basis for the diagnosis and treatment of IBD patients with abdominal pain in clinical practice.
