动脉粥样硬化(AS)是冠心病的病理基础,在AS进程中巨噬细胞数量增加和M1/M2型比例失衡是促使斑块不稳定的重要原因。因此,应用影像学技术定性定量监测巨噬细胞预防心血管事件的发生亟待解决。申请人前期国自然利用FD-OCT检测发现斑块稳定性与let-7b调节巨噬细胞自噬和极化密切相关。FD-OCT能定量检测巨噬细胞,但不能区分巨噬细胞亚型,而心脏核磁共振(CMR)分子成像技术能定性检测巨噬细胞,故本课题拟结合FD-OCT/CMR定性定量监测巨噬细胞评估斑块稳定性。预实验发现NEAT1在冠心病患者单核细胞中上调,可调控巨噬细胞自噬和极化。我们通过生物信息预测NEAT1可结合let-7b,推测NEAT1可能是AS进展中的关键分子靶点。本项目从临床出发,应用FD-OCT/CMR成像技术阐明NEAT1/Let-7b/TGF-βR1调控巨噬细胞自噬和极化的机制,为冠心病治疗提供新靶点和影像学支持。

Atherosclerosis (AS) is the pathological basis of coronary heart disease. The increase in the number of macrophages and the imbalance of M1 / M2 type ratio in AS process are important reasons for plaque instability. Therefore, the application of imaging technology to qualitative and quantitative monitoring of macrophages to prevent cardiovascular events needs to be solved. The applicant's previous National Natural Science Foundation of China used FD-OCT to find that plaque stability is closely related to let-7b regulating macrophage autophagy and polarization. FD-OCT can quantitatively detect macrophages, but it cannot distinguish macrophage subtypes. Cardiac magnetic resonance (CMR) molecular imaging technology can qualitatively detect macrophages. Therefore, this subject intends to combine FD-OCT and CMR to qualitatively and quantitatively monitor macrophages to assess plaque stability.Pre-experiments found that NEAT1 is up-regulated in monocytes of patients with coronary heart disease and can regulate autophagy and polarization of macrophages. We predict from biological information that NEAT1 can be combined with let-7b. It is speculated that NEAT1 may be a key molecular target in the progress of AS. This project is clinically based and uses FD-OCT / CMR imaging technology to elucidate the mechanism by which NEAT1 / Let-7b / TGF-βR1 regulates macrophage autophagy and polarization, providing new targets and imaging support for coronary heart disease treatment.
动脉粥样硬化(AS)是冠心病的病理基础,在AS进程中巨噬细胞的自噬、极化、M1/M2型比例失衡及铁死亡程度是促使斑块不稳定的重要原因,应用影像学技术定性定量监测巨噬细胞是预防心血管事件发生的重要手段。FD-OCT能定量检测巨噬细胞,但不能区分巨噬细胞亚型,而心脏核磁共振(CMR)分子成像技术能定性检测巨噬细胞。申请人前期国自然基于多种影像手段联合分析深入探索并阐明了let-7b靶向调控AS患者巨噬细胞极化的具体分子机制,为本项目的开展提供了基础。承接前期结果,本项目发现:NEAT1在AS患者和模型中显著上调,并通过let-7b/TGF-βR1信号通路调控巨噬细胞的极化方向,同时通过抑制pSTAT3的泛素化降解加剧巨噬细胞铁死亡,促进斑块不稳定;成功构建了M1型巨噬细胞靶向USPIO分子影像造影剂(USPIO-antiCD86),并实现动物模型中M1型巨噬细胞的在体检测;通过兔AS模型的建立及sh-NEAT1处理,结合FD-OCT和CMR影像学检测手段,发现斑块内M1型巨噬细胞比例及相关炎症标志显著下降,TCFA纤维帽厚度增加,管壁重构情况改善。另外在本项目支持的研究中发现AS过程中巨噬细胞的炎症反应收到甲基化酶mettl14的调控,同时mettl14还可以下调AS斑块内皮细胞NEAT1从而减弱内皮细胞焦亡减轻AS。综上,我们的研究以FD-OCT联合CMR为影像学检测手段,检测巨噬细胞极化方向和铁死亡水平变化,阐明了mettl14及NEAT1通过多条信号通路介导巨噬细胞自噬极化和铁死亡以及内皮细胞焦亡对AS病变的调控作用,为AS的诊治提供了新的靶点。
