基于“肾藏志”理论的肾虚证所致注意网络异常及神经机制研究

"Kidney storing Zhi" is main part of theory of kidney Zang-xiang, which is associated with attention. However, little is known about its neuropsychological mechanisms, which may affect the level of TCM treatment on related cognitive disorders. Attention networks are the main functional unit responsible for the attention. According to our previous research, in the current study, we employed attention network test as normal form and kidney deficiency syndrome from subhealthy group as research objects. We study attention network abnormalities caused by kidney deficiency syndrome and neuromechanisms from aspects of hormone levels, behavior, structure, resting state and task of MRI. Furthermore, we will apply the structural equation model to analysis mediation effect on brain structure and function, hormone levels, which could interpret how the changes of brain structure and function, hormone levels affect the path that kidney deficiency syndrome leading to abnormalities in attention networks. Finally, we analysis abnormalities in hormone levels, behavior, structure, resting state and task of MRI among the three subtypes of kidney deficiency syndrome (kidney Qi deficiency syndrome, kidney Yang deficiency syndrome and kidney Yin deficiency syndrome). Results of study will contribute to interpret attention neuromechanisms of "Kidney storing Zhi", and also have important theoretical significance and clinical value.

肾藏志是肾藏象理论心理内涵的重要组成部分，与注意关系密切，但相关的认知神经心理基础尚待深入阐明，并因此影响相关认知障碍疾病中医辨治水平。由于注意网络是负责注意的主要功能单元，因此本课题在前期工作基础上，应用注意网络测试范式，以亚健康人群中肾虚证（肾气虚、肾阳虚、肾阴虚）为研究对象，健康受试为对照，从行为学，结构、静息态与任务态的磁共振成像异常表征和内分泌激素水平三个维度，先后分层次研究肾虚证患者的注意网络异常表现、认知神经机制及生物学基础，并应用结构方程模型对大脑结构、功能异常表征和内分泌激素水平在肾虚证导致注意网络异常关系的中介效应进行分析，量化地阐明大脑结构、功能异常和内分泌激素水平变化在肾虚证导致注意网络异常中发挥作用的大小及相互之间的关系，最后比较肾虚证不同亚型之间的行为学、神经影像学、内分泌激素水平的差异。结果有助于阐明中医肾藏志的现代注意心理内涵，有重要的理论意义和临床意义。

肾藏志是肾藏象理论心理内涵的重要组成部分，与注意关系密切，但相关的认知神经心理基础尚待深入阐明，并因此影响相关认知障碍疾病中医辨治水平。为此，课题组应用核磁共振成像技术，选择亚健康人群中肾虚证为研究对象，健康受试为对照，以从行为学，结构、静息态与任务态的磁共振成像异常表征和内分泌激素水平三个维度，以功能与结构相结合的多模态分析方法，探讨肾虚证注意网络异常表现的神经机制，表达肾主志的神经心理内涵。该项目研究结果主要包括三部分：（1）确认肾虚证患者存在注意网络异常，注意网络中的执行控制功能异常，同时在即刻记忆、延时记忆、注意、视觉广度等方面也明显受损；（2）肾虚证患者的脑功能存在特异性变化，基于静息态和任务态功能磁共振研究发现包括左侧尾状核、左侧额中回、颞上回、左侧内侧和旁扣带脑回和右侧中央前回等脑区正激活，视觉网络内的功能连接减弱，视觉网络-背侧注意网络、视觉网络-额顶网络、感觉运动网络-额顶网络和腹侧注意网络-默认模式网络之间的功能连接减弱，并且与行为学数据相关；（3）肾虚证患者的脑白质结构异常，发现左颞横沟和左枕上回之间的白质纤维连接、左眶下沟和左枕颞内侧回的海马旁部分之间的白质纤维连接受损，并与行为学数据相关。研究结果将为确定肾藏志的注意心理内涵提供现代神经心理证据，为中医治疗相关疾病的注意损害提供理论上的依据，有重要的理论意义和临床意义。该项目的主体研究内容已按照课题计划书完成。
