阻塞性睡眠呼吸暂停综合征(OSAS)对儿童颌面部发育的影响日益受到家长和社会的重视。儿童OSAS面容是由腺样体和(或)扁桃体肥大以及全身多方面因素共同作用而产生的特征性面部特征改变,目前尚无针对OSAS儿童面容特点的量化研究报道。为了将传统基于临床主观判断的“腺样体面容”量化为依托先进技术手段的OSAS面容客观数据,本研究创新性的优化Dbid库人脸特征点,开发儿童侧面特征点系统,形成适用于OSAS儿童的人脸特征点标记识别技术;结合儿童颌面发育特点,量化提取并对比分析OSAS儿童和正常儿童面部的特征差异;通过不同临床区组OSAS儿童的随访面部特征比较,探索治疗转归中面部特征的纵向变化趋势。本研究量化阐释OSAS对儿童的面容特征发育的综合影响,并为OSAS的早期评估、筛查和预后评估提供客观量化参考,为儿童传统“腺样体面容”到“OSAS特征性面容”的临床观念转变发掘客观证据支持。

The influence of obstructive sleep apnea syndrome (OSAS) on children's maxillofacial development has been paid more and more attention by parents and society. Children's OSAS face has characteristic facial feature change caused by adenoid and / or tonsil hypertrophy and multiple factors of the whole body. At present, there is no quantitative research report on the face features of children with OSAS. In order to quantify the traditional "adenoid face" based on clinical subjective judgment as the objective face data of OSAS relying on advanced technology, this study innovatively optimizes the face feature points of the Dbid database, develops the side feature point system of children, and forms the face feature point mark recognition technology suitable for OSAS children. Combined with the characteristics of children's maxillofacial development, the quantitative extraction and comparative analysis of OSAS children and through the comparison of the follow-up facial features of OSAS children in different clinical area groups, to explore the longitudinal change trend of facial features in the treatment outcome. This study quantitatively explains the comprehensive influence of OSAS on the development of children's facial features, and provides objective quantitative reference for the early evaluation, screening and prognosis evaluation of OSAS, and explores objective evidence support for the transformation of children's traditional "adenoid face" to "OSAS characteristic face".
