椎间盘退变疾病是临床上的好发疾病,其病因复杂。众多研究显示椎间盘退变与终板的微循环障碍密切相关。终板微循环障碍引起局部缺血,影响了营养物质透过终板对椎间盘的供应,最终导致椎间盘退变。若能在终板下椎体的缺血阶段进行及时治疗,改善局部微循环,便有可能预防或延缓椎间盘退变的发生。血管内皮祖细胞(EPC)移植己在缺血性疾病的研究中体现出重要的价值,但尚未见EPC用于改善终板微循环,逆转椎间盘退变的相关研究。本研究采用自动轴向加压装置破坏兔终板的微循环建立兔终板微循环障碍及椎间盘退变的动物模型;穿刺收集兔的骨髓,体外分离、培养,获得兔骨髓来源的EPC,在体外荧光标记后自体移植,术后组织学观察EPC 在终板下缺血区域的分化、血管生成情况,MicroCT观察终板下微血管体积及数量的变化;同时MRI观察、组织学对照观察兔椎间盘退变的逆转情况,初步探索通过EPC移植防治椎间盘退变的相关机制。
英文摘要
Debenerative Disc Disease is frequently-occurring disease clinically.Its etiology is complex. Many Previous research results show that intervertebral disc degeneration was closely related to the microcirculation disorder in the area of subchondral bone of endplate. Local microcirculatory disturbance results in the ischemia of subchondral bone of endplate.Thus could result in the intervertebral disc nutrient deficiency and very likely lead to intervertebral disc cell death and disc degeneration. If the ischemic condition of the subchondral bone of endplate was treated timely and local microcirculation was improved subsequently, the process of disc degeneration may be prevented or reversed.The EPCs transplantation has provided a novel therapeutic strategy for the treatment of ischemic disease, however there seems to be no research report on the EPCs transplantation to improve the microcirculation of the subchondral bone and reverse the process of disc degeneration. In our study, the rabbit model of degenerated intervertebral discs is to be established, in which the microcirculatory disturbance of subchondral bone of endplate is induced by the equipment of controlled dynamic disc compression. Bone marrow aspiration of the rabbit model is to be done and bone marrow EPCs are to be isolated, cultured and labeled in vitro. After autologous EPCs transplantation is performed, the capability of differentiation and angiogenesis of EPCs is observed in the ischemic sub-endplate. The change of the microvessel density(MVD) in the ischemic sub-endplate is observed by micro-CT. Simultaneously,compared to the histological observation, magnetic resonance imaging(MRI) of the intervertebral discs degeneration and the degenerative disc reversion is to be observed. Our study is going to explore the mechanisms of autologous rabbit bone—marrow EPCs transplantation to reverse the process of disc degeneration.
