肠道微生物代谢参与动脉硬化斑块的发生发展，基于肠道微生物阐释冠心病相关的病理生理已成为当今医学研究的热点。本研究基于肠道菌群宏基因组学方法，探索前期疗效确切的心脾同治方（黄芪、当归、丹参、白术）对急性心肌缺血患者肠道菌群和血管新生的作用机制。临床研究采用心脾同治方治疗非ST段抬高型心肌梗死患者，从血管新生血清学、心脏核磁等方面评价心脾同治方的临床疗效，同时获得干预后患者的特异性肠道菌群种属；基础研究将特异性菌群移植到B6自交系无菌小鼠肠道后，制备急性心肌缺血模型，从血管生成蛋白PRKCD、HRG、ATP5B、ICAM1、CDH2等方面探索特异性菌群对缺血心肌血管新生的作用机制。本研究基于“心脾同治-肠道菌群-血管新生-缺血心肌”的治疗思路，评价心脾同治方调节肠道菌群改善NSTEMI患者及B6无菌小鼠缺血心肌血管新生的疗效，为阐释中医药调节肠道菌群治疗心血管疾病的药理机制提供新的科学依据。

The metabolism of gut microbiota is linked to the occurrence and development of coronary atherosclerosis plague. Finding from the latest literatures have identified specific gut flora played a protective role during the growth and rupture of plaque. However, how gut microbiota affects the myocardial angiogenesis induced by acute myocardial ischemia is not known. Our study wants to testify the formula of cardiosplenic concurrent treatment (invigorating the heart and spleen, nourishing and promoting the blood) modulate the gut microbiota of patients with NSTEMI and how specific gut microbiota affects the angiogenesis in rat with AMI. Our study comprised two parts. First, the genus of specific gut microbiota from patients’ stools treated with CSCT were identified with metagenome. Then the identified specific gut microbiota was successfully transplanted to the intestinal tract of B6 germ-free mouse. After that, the left anterior artery were ligated for 30min to establish the AMI model. Our study will evaluate the angiogenic role of specific gut microbiota through pathology, immunofluorescence, CMI, western blot, library of DNA. Combined with the latest research on intestinal flora, we present the new perspectives that the acute myocardial ischemia is caused by abnormality of heart and spleen in TCM theory, which provides new approaches to treat acute cardiovascular disease in TCM.
