过敏性猝死在法医学实践中很常见,但死后法医学诊断却很困难。尤其是用药过程中发生的猝死,鉴别过敏性猝死抑或冠状动脉性猝死是法医病理学家面临的棘手问题。而代谢组学是通过分析疾病状态下的代谢物谱,发现疾病的生物标记物而进行疾病的诊断,因此可作为过敏性猝死诊断的技术手段。另外,BNP的分泌不但与心室功能有良好的相关性,也可证明室性心律失常的存在,从而阐明冠状动脉性猝死的机制。我们的前期研究也发现,与对照组和过敏性猝死相比,急性心肌缺血大鼠的BNP明显升高。因此,本实验拟建立大鼠过敏性猝死、冠状动脉性猝死模型,并采取过敏性猝死、冠状动脉性猝死人体标本,采用LC-MS结合1H NMR技术分析其血和尿中的差异代谢物,检测其血及心肌的BNP、BNPmRNA水平,从代谢组学角度和基因水平探寻过敏性猝死和冠状动脉性猝死的特异性生物标志物,为过敏性猝死的诊断及其与冠状动脉性猝死的鉴别诊断提供可靠依据。
英文摘要
Sudden death from anaphylaxis is common in medicolegal expertis. Especially,it is the troublesome problem to identify Sudden death from anaphylaxis or sudden coronary death happened during treatment for forensic pathologists.While metabolomics, by analyzing the metabolite spectrum of disease states, the specific metabolite spectrum can be found for disease diagnosis. In addition, the BNP is not only relative with the ventricle function correlation, but also can prove the existence of ventricular arrhythmia, clarify the mechanism of sudden coronary death. Our previous studies have showed that the level of BNP protein and BNP mRNA is increased in early myocardial ischemia but not in anaphylaxis. Therefore, this experiment will establish rat model of Sudden death from anaphylaxis and sudden coronary death, and take some human samples of sudden death from anaphylaxis and sudden coronary death. And specimens were analyzed by LC-MS combined with 1H NMR metabolomics to find differential metabolites in the blood and urine metabolites spectra, to detect the changes of BNP and BNPmRNA in cardiomyocytes and myocardium, find out the specific biomark of allergic sudden death on the metabolomics and gene level, provide a reliable diagnosis biomark for diagnosis and differential diagnosis of sudden allergic death from sudden coronary death.
