我国人口众多,医疗资源紧缺且分布不平衡。合理发展和利用“互联网+医疗健康”背景下的线上医疗服务可以垂直整合优质医疗资源,通过线上线下协同的方式下沉到基层与社区,改善医疗资源配置的不平衡,提高基层医护人员和医疗机构的业务水平和服务质量。在当前疫情背景下,发展互联网诊疗可以将病人合理分流,以线上的形式让病人足不出户地得到诊断与治疗,减少医院人群交叉感染,减轻医护人员负担。本课题将聚焦基于社区的健康管理的应用场景,考虑三级医院与社区医院组成医联体内的线上线下诊疗分配决策与资源配置优化。本课题将综合数据分析、随机过程与控制等理论探究三个科学问题:1)线上诊疗对病人健康状况以及对后续医疗资源使用率产生什么影响?2)在有限门诊资源下如何将社区所病人群体分配至最优诊疗方式?3)医联体如何优化配置线上线下门诊资源?本课题旨在通过线上+线下的方式垂直整合医疗资源优化配置,实现社区+三级医院1+1>2的效果。
英文摘要
As the most populated country in the world, China faces challenges in its health care system: medical resources are scarce and distributed unevenly across the nation. Through the development and adoption of online health care services, medical resources can be integrated and efficiently utilized to mitigate the unbalance. In China’s tiered health care system, tertiary care can be delivered to communities through telemedicine, which will improve the quality of primary care and services in the communities. Amid the COVID-19 pandemic, using telemedicine could reduce crowding in hospitals and infections therein; and it can also ease the burden of health care providers while the fight the pandemic. This research focus on community-based health management, in which a tertiary hospital and multiple community hospitals form an organization to maintain the health of the panels of patients. We develop models and algorithms based on data analytics, stochastic processes, and dynamic programming to address three key questions. 1) how does telemedicine impact the patient’s health and utilization of healthcare resources? 2) under limited capacity, how to optimally allocate patients to different appointment modes? 3) within a health organization, how to balance and allocate medical resources through telemedicine? This research aims to answer the three research questions to optimally allocate and utilize medical resources leveraging telemedicine, and ultimately make primary care + tertiary care “1+1>2” through telemedicine.
