基于动态、准确的数据来综合地评价慢性病的疾病负担研究对正确制定卫生政策、合理配置卫生资源具有重要的现实意义。疾病负担包含了发病、死亡、残疾、生活质量下降以及经济损失等多方面内容。目前尚未见对云南慢性病疾病负担进行以上内容的综合评价报道。国内疾病负担研究大多数通过横断面调查获得自我报告患病率来计算后续评价指标，存在较大的报告偏倚和回忆偏倚，并且缺乏规模较大且固定的现场，难以对其发展趋势进行分析。因此，本研究拟构建一个涵盖疾病发生发展特征、疾病致寿命损失、疾病致劳动能力损失、疾病致经济损失、疾病致生命质量损失等五方面内容的模型，并且利用西南区域自然人群队列研究项目在云南建立的规模较大且固定的队列现场广泛采集相关数据资料，运用所建模型对云南省农村居民慢性病疾病负担进行综合评价，为合理分配有限的卫生资源、探求适用于云南农村地区的慢性病患者社区干预模式以及制定慢性病管理政策提供重要科学依据。

It can not only scientifically and truly reflect the health status and degree of rural residents, but also has important practical significance for correctly formulating health policies, meeting health needs and rationally allocating health resources to comprehensively evaluate the burden of chronic no-communicable diseases (CND) based on dynamic and accurate data. The burden of disease includes various contents: morbidity, mortality, disability, decline of quality of life and economic losses. At present, there were not related studies to be reported in the aspect on comprehensive evaluation of the burden of CND in Yunnan province. In most domestic studies, the self-reported prevalence were obtained through cross-sectional survey and it was used to calculate the other evaluation indexes. There were more report bias and recall bias in these studies and the data were not as accurate as the data provided by the cohort study. Moreover, a large and fixed study side was lacking in the burden of disease studies in China. So it is difficult to analyze the development trend of the burden of disease. In view of the above deficiencies in the previous research for burden of disease, the aim of this study is to construct the comprehensive evaluation model which contains five aspects, including occurrence and development of diseases, loss of life caused by diseases, loss of labor ability caused by diseases, economic loss caused by diseases, and loss of quality of life caused by diseases to evaluate the burden of CND. And then to apply this model to carry out the comprehensive evaluation of the burden of CND for the rural residents in Yunnan province by widely and proactively collecting the relevant data based on a cohort research project, southwest region natural population cohort research project, in which there is a large-scale and fixed cohort study site in Yunnan province. The research results will provide an important and scientific basis for the rational allocation of limited health resources, for the exploration of community intervention models for patients with CND and for the formulation of CND management policies in rural areas of Yunnan province.
