Brachial plexus avulsion (BPA) induced neuropathic pain (NPP) is one intractable complication in clinical work. Pain sensitization plays a crucial part in the occurrence and maintenance of NPP. The activation of BDNF-TrκB cascade on somatosensory pathway is a rapid reaction of this pathway under pathological conditions. The abnormal expression and function of KCC2, as the most important intracellular K+-Cl- ion combined transporter, can promote the occurrence and development of NPP. Previous studies showed that the glial cells activated in the spinal dorsal horn after BPA can act on KCC2 through the cascade of BDNF-TrκB, resulting in intracellular Cl- accumulation of neurons, impairing central inhibition of the spinal cord, and finally leading to pain sensitization. Then, as the first stop of the integration of somatosensory information, dorsal root ganglion should be affected by this cascade reaction in the process of pain information processing and interact with the spinal cord, which plays an equally important role in the process of pain sensitization. Our research group has successfully established a new BPA induced NPP mouse model in the early stage. On this basis, it intends to further explore the deep mechanism of pain sensitization on the somatosensory pathway based on the loss of inhibitory function caused by the cascade of BDNF-TrκB on KCC2, so as to provide theoretical basis for using peripheral nociceptors as therapeutic targets for NPP in the future.
臂丛神经根性撕脱伤(BPA)是上肢周围神经损伤中最严重的类型。BPA发生后,其所带来严重神经病理性疼痛(NP)并发症不光大大降低了患者的生活质量,也使该类患者的治疗更加复杂化。对于BPA诱发NP的发生机制缺乏了解是制约该类疼痛治疗的主要因素。在本研究中,我们关注外周水平钾-氯离子协同转运蛋白(KCC2),深入剖析其在外周水平参与疼痛及相关痛觉敏化的分子机制。我们使用课题组原创设计的新型臂丛神经中干(C7)撕脱小鼠模型进行研究。BPA撕脱伤后1 d,小鼠撕脱同侧前足及后足即表现出明显的机械痛敏,直至伤后28 d;同时,CatWalk步态分析检测结果显示BPA损伤对小鼠运动功能无影响。以BPA后7 d为时间点,使用Western blot和荧光原位杂交(FISH)检测BPA小鼠DRG内KCC2表达明显下降。于外周DRG内过表达KCC2,可在逆转DRG内神经元过度兴奋的同时缓解BPA小鼠撕脱同侧前足和后足的机械痛敏行为。通过使用脊髓光纤钙成像,我们观察到撕脱同侧颈段脊髓背角(SDH)的神经元钙活动亦可被过表达KCC2所减小。通过外周DRG水平过表达KCC2,DRG内的BDNF合成和向相应SDH的释放也可被抑制,而BDNF曾被报道为KCC2的上游分子。除此之外,来自临床BPA患者的标本结果显示,BPA患者撕脱神经根内BDNF及其受体TrkB的表达升高,KCC2的表达降低,这与上述BPA小鼠的结果一致。本研究证实了外周水平伤害感受器内KCC2的表达并参与BPA诱发的NP的机制,并于外周-中枢(DRG-SDH)环路水平验证,以外周KCC2为分子靶点进行特异性调控,在镇痛的同时,亦可逆转BPA诱发的相应颈段脊髓的中枢敏化状态,且这种逆转作用是通过减少外周BDNF的合成与释放实现的。另外,人源标本中核心分子表达变化与BPA小鼠的一致性为本研究成果的后续临床转化打下了良好的理论基础。
