既往研究显示肿瘤遗传咨询有利于改善人们对于肿瘤风险因素的认识，改变筛查的行为。但是我国缺乏肿瘤遗传咨询相关的研究，肿瘤咨询对于家族性乳腺癌患者及其家属的社会心理影响尚不明确，对于患者免疫细胞功能的影响和预后的影响仍是未知。本课题主要是针对家族性乳腺癌的家庭，建立标准的遗传咨询体系和流程。已收集70例BRCA1/2突变患者的信息。通过比较乳腺癌遗传咨询前后家族性乳腺癌患者及其家属的心理弹性和社会支持的变化，以及乳腺癌患者外周血免疫细胞的变化，研究遗传咨询在乳腺癌患者及其家属社会心理方面的影响。探讨乳腺癌遗传咨询是否可以通过改变乳腺癌患者及其家属的社会心理因素提高患者的免疫功能从而提高预后。同时对接受了遗传咨询的家族性乳腺癌患者的随访进行依从性分析，是否可以提高患者及其家属早筛早治，从而提高其生存。

Previous studies have shown that genetic counseling can help people to understand the risk factors of cancer and make them change the screening behaviors. However, there is a lack of research on genetic counseling for familial cancer in China. The psychosocial impact of genetic counseling on familial breast cancer patients and their families is not clear, and the effect on patient’s immune cell function and their prognosis is still unknown. This study is aimed to establish a standard genetic counseling system and procedures for familial breast cancer patients and their relatives. We already collected 70 patients with BRCA1/2 mutations. By comparing the changes of psychological resilience and social support of familial breast cancer patients and their families before and after genetic counseling, the change of immune cells in peripheral blood of patients, to find the influence of genetic counseling on the social psychology of breast cancer patients and their families. Also to explore whether genetic counseling for familial breast cancer can improve the prognosis of breast cancer patients and their families by changing their psychosocial factors and immune functions. At the same time, we take the compliance analysis of familial breast cancer patients and relatives on their follow-up, to make sure whether it could improve the early screening and early treatment of patients and their families, in order to improve their survival.
