乳腺癌患者认知功能受损发生率高，严重影响其社会心理康复，亟需有效的心理干预措施。已有证据显示正念干预效果较好，但传统方案卫生资源经济性不足、干预效果缺乏临床意义考量及机制探讨。本项目在前期引进乳腺癌患者认知功能受损评估量表FACT-Cog及正念干预研究基础上，拟采用质性及量性研究方法，借助移动互联网技术及分子生物学手段，开展以下研究：1）以正念干预改善认知功能受损的有效成分（聚焦注意、开放监控练习）为核心，利用移动互联网技术构建精准自助式正念干预方案；2）综合分布法和锚定法制定FACT-Cog最小重要性差值，并结合该差值评价干预效果；3）以正念疗法降低应激反应为切入点，从心理和生理层面，解析正念干预改善患者认知功能受损的机制。本项目是首次针对乳腺癌患者认知功能受损进行适宜性、自助式正念干预方案研究，有助于形成经济、高效的乳腺癌患者心理健康管理策略，为促进乳腺癌患者社会心理康复提供实践指导。

Cognitive impairment is prevalent in patients with breast cancer, which prevents the psychosocial rehabilitation of the patients. It is urgent to explore effective psychological interventions to improve cognitive impairment of the patients. Mindfulness-based intervention was evidenced to be an optimal intervention, however, the implementation of traditional mindfulness-based intervention depends on professional psychological health resources, and the clinical implications and mechanisms of the intervention are unclear. We previously assessed the psychometric properties of the Functional Assessment of Cancer Therapy-Cognitive Function (FACT-Cog) in Chinese patients with breast cancer and explored the efficacy of a mindfulness-based intervention for their negative emotions. We propose a research on mindfulness-based intervention for cognitive impairment with qualitative and quantitative research methods, technology of mobile internet, and molecular biology experiments. We will 1) develop a targeted and self-help intervention with exercises of focused attention and open monitoring, taking advantage of the technology of mobile internet; 2) assess the effectiveness of the intervention and evaluate its clinical implications using the minimally important differences of the FACT-Cog; 3) explore the mechanisms of the intervention through examining the effect of psychological and physical stress responses. This project is the first one to focus on the suitable and self-help mindfulness-based intervention for cognitive impairment in patients with breast cancer. The results will help to develop an economic and efficient management strategy for psychological health of the patients, and will guide the practice on psychosocial rehabilitation of patients with breast cancer.
