云南省大姚县因受青石棉污染,是全球少有MPM高发区。目前MPM诊治是世界上公认的难题。我们前期研究发现,常规影像学虽能发现病灶,提示部分病理改变,但早期诊断敏感性及特异性均较低。研究认为MPM肿瘤细胞膜蛋白间皮素(MLSN)在正常人不表达或极低表达,而在MPM早期即可增高,且与肿瘤迁移、侵袭性密切相关,但其演变过程和规律仍不清楚。本研究拟培养4种MPM细胞系(永生化间皮细胞、MLSN低表达、高表达、MLSN基因敲低),建立裸鼠异位种植模型;构建MLSN靶向超顺磁性纳米探针,在MPM不同时期行体内外多模态MRI扫描,采用分子影像学方法检测肿瘤内MLSN表达;探索早期MPM演变过程;研究MPM的迁移和侵袭规律;期望建立一种更准确诊断早期MPM的新方法,为临床早期发现MPM、靶向治疗及预后评估提供新的理论和实验依据。

Due to the pollution of blue asbestos, Dayao County, Yunnan Province is one of the few areas with high MPM in the world. The current diagnosis and treatment of MPM is a recognized problem in the world. Our previous study found that although conventional imaging can detect lesions, suggesting partial pathological changes, but its early diagnosis sensitivity and specificity are low. It is considered that MPM tumor cell membrane protein mesothelin (MLSN) is not expressed or very little in normal people, but it can be increased in the early stage of MPM, and is closely related to tumor migration and invasiveness, but its evolution process and regularity are still unclear. This study intends to culture four MPM cell lines (immortalized mesothelial cells, MLSN low expression, high expression, MLSN gene knockdown), establish a heterotopic implant model in nude mice; construct MLSN targeted superparamagnetic nanoprobes in MPM Multi-modal MRI scans in vitro and in vivo were performed in different periods. Molecular imaging was used to detect the expression of MLSN in tumors. The evolution of early MPM was explored. The migration and invasion of MPM were studied. It is expected to establish a new method for more accurate diagnosis of early MPM. Early clinical discovery of MPM, targeted therapy and prognostic evaluation provides new theoretical and experimental evidence.
