阿尔茨海默症（AD）易防难治，将预防关口前移，对其早期阶段轻度认知障碍（MCI）开展一级预防，是减轻未来AD疾病负担的最根本策略。而社区作为第一道防线，目前尚不具备充分的MCI预防与高危人群管理能力。本项目引入Triangle模型，提出从社区老年人MCI风险评估，到开展以社区为基础、风险为导向的老年人群精准管理方案，创新地提出根据风险等级及来源对老年人进行分层分级管理，对风险等级较高且可控性强的老年人进行优先重点干预，从而构建社区老年人群的MCI风险分层分级管理模型，力求在有效降低社区老年人MCI的发病风险的同时节约卫生资源。本项目创新地将Triangle模型应用于认知障碍疾病的社区一级预防，研制符合我国特定社会文化背景的MCI风险评估工具，并构建社区老年人群MCI风险分层分级管理模型，这些成果可助力突破MCI社区精准预防的瓶颈，有助于推动社区AD一级预防能力建设。

Mild cognitive impairment (MCI) has been considered as the early stage of Alzheimer disease (AD), thus making primary prevention of MCI a top priority is the most essential strategy for reducing AD disease burden in the future. However, as the first line of defense, communities are weak in the capacity of MCI prevention and management of people at-risk. The study aims to build the model of management strategy for old people who are at MCI risks in a cost-effective way. In the study, based on the Triangle model, a community-based risk-oriented management strategy, which includes MCI risk evaluation and analysis at first, followed by precise lifestyle intervention according to the results of risk analysis, is developed. The strategy is also hierarchical, that is, individuals with higher and modifiable risk of MCI would be a priority. Outcome of this study includes a MCI risk assessment tool and a model of hierarchical management of old people at-risk of MCI. The study which provides effective measures to precise prevention of MCI at community level is meaningful to the establishment of community capacity for prevention of cognitive function related diseases.
