电针足少阳经穴经TGF-β1/Samd通路调节ACL本体感觉的机制研究

膝关节前交叉韧带(ACL)本体感觉与其稳定性密切相关。当本体感觉受损时,关节稳定性会受到影响。TGF-β1/Samd通路在神经系统损伤修复、再生中起着重要作用。当ACL损伤后,ACL神经元中TGF-β1表达下降,Samd-2、Smad-3、MMP-2、MMP-9表达上调,本体感觉减退。《黄帝内经》曰:“少阳为枢......枢折,即骨繇而不安于地。故骨繇者取之少阳”,表明本体感觉受损所致膝关节不稳与足少阳胆经息息相关。电针疗法能增加运动神经元活性,促进神经再生,调节本体感觉,但机理尚未明确。因此,我们提出科学假说:基于“少阳主骨”和“循经取穴”理论,电针足少阳经穴可以调控TGF-β1/Samd通路,恢复ACL受损的本体感觉。研究以SD大鼠ACL本体感觉障碍模型为对象,采用神经电生理学、组织病理学及分子生物学等方法,研究作用机制,提供一种有效的治疗方法,丰富和发展经络理论的科学内涵。
英文摘要
The proprioception of anterior cruciate ligament (ACL) is closely related to its stability. Proprioception disturbance of ACL would lead to instability of the knee. TGF-β1/Samd signaling pathway plays an important role in the damage repair and regeneration in the nervous system. When ACL was injured, the expressions of TGF-β1 decreased and expressions of Samd-2、Samd-3、MMP-2、MMP-9 increased in proprioceptive neurons, leading to the proprioception hypoesthesia in ACL. “Yellow Emperor’s Cannon of Medicine” recorded that “Shaoyang is responsible for pivoting……If the activity of pivoting is broken, bones will become weak and the patient cannot stand stably on the ground. So bone weakness can be treated by needling the Acupoints located on the Shaoyang Channel”, demonstrating that the knee instability caused by the proprioception hypoesthesia is closely linked with Gall bladder Meridian of Foot-shaoyang. Electroacupuncture can increase motor neuron activity, promote nerve regeneration, and improve the injured proprioception, though the mechanism of it is not yet clear. Therefore, this study assumes the scientific hypothesis that according to the theory of traditional Chinese medical science “Shaoyang dominates bone” and “Acupuncture the points along the meridian”, electroacupuncture on Foot-Shaoyang Meridian Points can regulate the TGF-β1/Samd signaling pathway to restore the damaged proprioception of ACL. The study takes the ACL Proprioception Disturbance model in SD rats as the object and the mechanism is researched by methods including electrophysiology, histopathology, and molecular biology. A new treatment and intervention approaches can be established and the scientific connotation of TCM acupuncture theory is enriched and developed.
