局灶节段性肾小球硬化(FSGS)对激素等各种治疗方案反应差,易快速进展为终末期肾病。深入探讨FSGS发病的始动机制是探索新治疗方法的前提和关键所在。遗传因素是FSGS发病的重要原因。我们前期在一个FSGS家系中筛选到一个新的候选致病ARHGAP32。其在FSGS肾组织中表达明显增加。我们又在309例散发FSGS患者发现了2个新突变。我们建立了ARHGAP32基因Leu754Val点突变小鼠,研究发现,突变小鼠对阿霉素损伤的敏感性显著增加。该突变位点在不同物种间高度保守,突变可能导致其GAP功能域异常,影响Rho GTPase的活性,通过干扰细胞骨架形成,导致足细胞损害和FSGS发生。本项目拟通过细胞实验,动物实验,及突变小鼠肾病模型肾小球单细胞表达谱的研究,确立ARHGAP32基因在FSGS发病中的地位,阐明Leu754Val突变导致FSGS的机制,为临床诊断提供依据,为治疗探索提供方向。

Due to its poor response to glucosteroids and other treatment options, focal segmental glomerulosclerosis (FSGS) is prone to rapid progression to end-stage renal disease. Investigation of initial pathogenesis of FSGS is the premise and key point to explore new treatment. Genetic factors play an important role in the pathogenesis of FSGS. Previously, we identified a new candidate pathogenic gene ARHGAP32 in a FSGS pedigree. The arhgap32 expression in renal tissue of FSGS patients increased significantly. We screened two new possibly pathogenic mutations of ARHGAP32 in 309 patients with sporadic FSGS. We established a mouse model with leu754val point mutation of ARHGAP32 gene, and found that the sensitivity of the knockin mice to adriamycin damage increased extremely. The leu754val mutation is the highly conserved among different species.It is speculated that the mutation could lead to the abnormal function of GAP domain, affect the activity of Rho GTPase, and cause podocyte damage and FSGS by interfering with cytoskeleton formation. This project aims to clarify the role of ARHGAP32 gene in the pathogenesis of FSGS, illustrate the mechanism of leu754val mutation leading to FSGS, by in vitro cell experiments, in vivo animal experiments and single cell transcriptome sequencing of glomerulus in the mutant mouse nephropathy model. It would be helpful for clinically genetic diagnosis and future treatment exploration.
局灶性节段性肾小球硬化(FSGS)对激素治疗反应差,是终末期肾病的重要病因。遗传因素在FSGS发病中起关键作用。本研究基于前期在一个FSGS家系中发现的新型候选致病基因ARHGAP32及其L754V突变(309例散发患者中验证2例新突变),通过多层面实验阐明该突变的致病机制及科学意义。.研究围绕“基因筛查—功能验证—机制解析”展开,结合临床样本分析、生物信息学预测、体外细胞模型(人足细胞、COS-7细胞)、CRISPR/Cas9构建的Arhgap32 L754V点突变小鼠模型及分子互作分析,系统探索突变致病的分子机制。结果表明:(1)ARHGAP32 L754V突变为条件致病突变,家系中8名携带者均表现肾功能异常,生物信息学工具(SIFT、PolyPhen-2)及病例对照分析支持其致病性;(2)突变导致足细胞标志物nephrin、synaptopodin表达显著降低,F-actin骨架破坏,并通过等温滴定量热法(ITC)证实突变增强Arhgap32与Cdc42的亲和力(KD值降低20倍),抑制Rho GTPase(Rac1、RhoA、Cdc42)活性,最终引发足细胞损伤;(3)突变小鼠在阿霉素诱导下出现蛋白尿、肾功能恶化及典型FSGS病理表型(系膜增生、足突融合),验证了突变与环境因素的协同致病效应。此外,项目还发现TMEM30A通过抑制NLRP3焦亡保护足细胞,其敲除导致肾小管葡萄糖吸收减少,揭示了其在足细胞病中的潜在治疗价值。.本研究阐明ARHGAP32 L754V突变通过Rho GTPase信号失调导致FSGS的分子机制,为遗传诊断和治疗探索提供了理论依据。.项目成果:发表SCI论文6篇、中文论文4篇,申报专利2项(转化1项,金额210万元),培养硕士生1名、博士后1名,并在国内外学术会议报告4次。研究建立的基因编辑模型和机制解析方法为FSGS的深入研究奠定了一定基础。
