颈椎病是常见的脊柱退行性疾病,颈前路椎间融合术(ACF)是治疗颈椎病的标准术式。但研究发现ACF术后出现了邻椎病,年发生率达2%-2.9% ,二次手术率达7%至66%。ACF术后邻椎病的发生已成为外科医生的巨大挑战。其发生与手术相关还是自然退化为主因,存广泛争议。ACF术后整个颈椎的在体三维活动代偿能力尚不清。我们采用口腔颌面锥形束CT(CBCT)结合3D-3D图像配准精确测量颈椎在体三维运动,误差小于0.1mm。单节段、双节段、三节段ACF术患者,术前和术后行7个功能体位CBCT扫描、三维建模、3D-3D图像配准,精确测量三维运动和形态学参数,结合MRI扫描,为ACF术对邻近节段在体生物力学影响提供准确数据,为解决ACF术对邻椎病发生的争议提供客观依据。精确测量ACF术后整个颈椎的三维运动代偿能力,为多节段颈椎病制定ACF术方案提供科学指导,为颈椎非融合新型技术的研究提供参照依据。

Cervical spondylopathy is one of the most common spinal degenerative diseases, and anterior cervical interbody fusion (ACF) is the standard surgical treatment. Lots of clinical studies reported that adjacent segments degenerative disease (ADS) often occurred after ACF operations, the annual incidence rate was 2% - 2.9%, and 7% to 66% ADS need secondary surgery. The occurrence of ADS after ACF has become a great challenge to spine surgeons. However there has been widely controversial if ADS is mainly related to surgery or natural degradation process. .We have innovatively applied the international advanced technology of CBCT combined with 3D-3D image registration technology to accurately measure the three-dimensional motion of cervical spine under seven functional postures. The allowable error of the experimental method is less than 0.1 mm. Single, double and three-level ACF subjects were scanned by thin-slice CBCT in 7 functional positions before and after operation. After three-dimensional modeling, 3D-3D image registration was completed using volume fusion technology.The three-dimensional motion and morphological parameters of adjacent segments were measured accurately. Combined with MRI scanning, accurate data were provided for the biomechanical effects of ACF on adjacent segments, and objective basis was provided for resolving the controversy of ACF on adjacent vertebral diseases. Accurate measurement of the three-dimensional motion compensation ability of the whole cervical spine after ACF can provide scientific guidance on how to perform ACF for multi-level cervical spondylosis, and provide a reference basis for the research of new non-fusion techniques of cervical spine.
