口腔慢性移植物抗宿主病(Graft-versus-host disease, GVHD)是异基因造血干细胞移植后常见的严重并发症,准确评估疾病病程及活动性是提高患者生活质量和预后的关键。目前临床根据患者症状及体征制定了疾病活动性评估表,但已有研究表明该量表并不能有效反映患者疾病严重程度。前期研究发现,超声多普勒血流信号可优于临床判断口腔慢性GVHD疾病活动性。另外,Th2细胞因子异常表达参与口腔黏膜淋巴细胞浸润和疾病进展。本课题拟以口腔慢性GVHD大鼠模型为研究对象,采用超声造影定量分析观察慢性GVHD大鼠口腔黏膜及黏膜下组织微循环灌注,测定血清Th2细胞因子表达水平,以病理为金标准动态追踪不同病程口腔慢性GVHD病理变化规律和超声造影定量参数、Th2细胞因子水平的相关性。项目预期研究为准确评估口腔慢性GVHD病程和活动性探索影像学和血清学指标,对临床适时调整治疗方案改善预后具有重要意义。

Oral chronic graft-versus-host disease (GVHD) is a serious complication that commonly occurs after allogeneic hematopoietic stem cell transplantation. Accurate evaluation of disease course and activity is necessary to improve patients' quality of life and prognosis. Until recently, the evaluation of disease activity is based on the clinical symptoms and signs of patients. However, the clinical experience suggests that the present response criteria lack validity in depicting the severity of the disease. In our previous studies, we demonstrated that Doppler blood flow signals detected the onset of disease activity before improvement of clinical objective measurement occurred. Furthermore, abnormal expression of Th2 cytokines was involved in lymphocytic infiltration and disease progression. Oral chronic GVHD rat models will be involved in our study, quantitative analysis of contrast-enhanced ultrasound will be used to evaluate the changes of microcirculation perfusion in oral mucosa and submucosa and the expression level of Th2 cytokines in serum will be measured. In addition, dynamic changes of pathological with the corresponding parameters of contrast-enhanced ultrasound and Th2 cytokines level will be explored. The expected results of this project will be important to provide imaging and serological criteria for the assessment of oral chronic GVHD, and is of great significance for clinical timely adjustment of therapeutic schemes to improve prognosis.
