壮医药线点灸联合针挑疗法通过AngII-NOX-ROS轴治疗sciatica的效果及机制研究

坐骨神经痛对人们身体健康和生活质量造成极大影响。近期研究表明血管紧张素 II（AngII）-NADPH氧化酶（NOX）-活性氧簇（ROS）轴介导氧化损伤γ-氨基丁酸（GABA）能神经元在坐骨神经痛发生中有重要作用。课题组在前期临床工作中采用壮医药线点灸联合针挑疗法治疗坐骨神经痛患者，观察到治疗后患者AngII水平、疼痛评分等降低；在前期的动物实验中采用该疗法干预坐骨神经慢性压迫损伤疼痛模型大鼠，注意到模型大鼠干预后NOX4 mRNA 表达下降、ROS水平下降而抗氧化能力升高，并抑制GABA能神经元凋亡。课题组设想该疗法治疗坐骨神经痛是通过调控AngII进而调节ROS，改善ROS对GABA能神经元氧化损伤而显效。为证实此设想，本课题在前期工作基础上，从动物实验和临床实验两方面进一步开展研究工作，研究结果有助于揭示该疗法治疗坐骨神经痛的分子生物学机制，同时也有利于促进民族医学基础研究和交流。

Sciatica has a strong impact on people’s health and the quality of life. Recent studies have shown that angiotensin II（AngⅡ）-NADPH oxidase（NOX）-reactive oxygen species（ROS）axis mediates oxidative damages of Gamma-aminobutyric acid（GABA）ergic neurons（GABAergic neurons）,which plays an important role in sciatica . In our early clinical research , our group found that the patients with sciatica whose levels of AngⅡ and pain score reduced after the therapy of combined medicated thread moxibustion with needle picking of Zhuang nationality medicine. In our early animal experiment our group also noticed that the expression of NOX4 mRNA and the level of ROS reduced while the antioxidant ability increased and inhibited the apoptosis of GABAergic neurons in the rats with chronic constriction injury of sciatic nerve after administrated with the therapy of combined medicated thread moxibustion with needle picking of Zhuang nationality medicine, so our group supposed that the therapy of combined medicated thread moxibustion with needle picking of Zhuang nationality medicine took an effect by regulating the level of AngⅡ and then mediating the level of ROS to alleviate the oxidative damages of GABAergic neurons. To verify this hypothesis, based on our early research results our group will continue further studies through the animal experiment and the clinical research. Our findings are expected to discover the mechanisms of the therapy of combined medicated thread moxibustion with needle picking of Zhuang nationality medicine on the treatmen of sciatica,meantime , to be helpful for the development and communication of basic research of ethnomedicine.
