声带良性病变的发病率高、患病群体大,严重影响患者的生活质量。目前声带良性病变的临床防治手段有限并存在较多制约。半封闭声道训练(SOVTE)是以声道远端部分封闭为特点的嗓音训练方式,近年来已应用在嗓音疾病的辅助治疗中。我们前期通过离体模型证实SOVTE可优化声带振动时的空气动力学,改善嗓音功能;并在临床研究中发现SOVTE可有效提高发声效率,预防嗓音疲劳,但其在声带急性损伤及声带修复过程中的作用和机制尚不明确。据此我们提出假设:SOVTE可通过细胞生物力学机制抑制炎症反应、影响细胞外基质合成以预防声带急性损伤,并可在声带修复过程中减少瘢痕愈合,促进组织结构重塑。本实验拟采用细胞生物力学模型、在体吹鸣模型等手段,观察其对声带成纤维细胞中炎症因子、纤维化通路及组织结构的影响。本项目有望揭示声带振动优化模式及SOVTE的生物力学机制,为临床指导正确用嗓及创伤后修复提供更充分的科学依据。
英文摘要
The incidence of benign vocal fold lesions is high and the affected population is large, which seriously affects the life quality of patients. At present, the prevention and treatment of benign vocal fold lesions are limited. Semi-occluded Vocal Tract Exercise (SOVTE) is one kind of voice training method that is characterized by the partial occlusion of the distal end of the vocal tract. Using vitro models, we have confirmed that SOVTE can optimize aerodynamics and increase physiological functions during vocal fold vibration. In clinical studies, we have found that SOVTE can effectively improve vocal efficiency and prevent the occurrence of vocal fatigue. However, its role and mechanism in the process of acute vocal fold injury and vocal fold wound healing are unclear. Therefore, we propose the hypotheses: based on the cell biomechanical mechanism, SOVTE can inhibit the inflammatory response, affect the synthesis of extracellular matrix to prevent acute vocal fold injury, and prevent from vocal scar and promote tissue remodeling during the vocal fold repair process. Using cell biomechanical models and in-vivo phonation experiments, this project aims to observe the effects of SOVTE on inflammatory indicators, fibrotic pathways in fibroblasts and vocal tissue structure. This project is expected to reveal the optimizing mode of vocal fold vibration and the biomechanical mechanism of SOVTE and provide a more scientific basis for clinical guidance in the correct use of voice and post-traumatic repair.
