糖尿病周围神经病变(DPN)发病机制复杂。雪旺细胞(SCs)增殖、凋亡与周围神经损伤修复密切相关。课题组完成的上个国家自然基金面上项目,进行了活血化瘀通络法对自发性2型DPN大鼠炎症JNK/SAPK通路的调控作用及机制研究,发现体现该治法的中药通络糖泰(TLTT)可改善炎症、氧化应激,抑制DPN大鼠坐骨神经JNK信号通路,对高糖环境下SCs增殖与凋亡调控作用并存。最新研究发现Wnt/β-catenin和PERK-CHOP-Caspase-12信号转导通路与SCs增殖、凋亡密切相关,根据前期研究结果,推测TLTT调控作用与此相关,故本次研究拟行体内、体外实验,运用神经电生理、透射电镜、RT-PCR、免疫荧光组化、ELISA、MTT测定法及流式细胞术,建立SCs与神经元共培体等方法,研究TLLT可能作用微观机制,从全新角度探索该法治疗DPN的现代科学本质,为中医药治疗DPN开辟新的思路与方法。

As the pathogenesis of diabetic peripheral neuropathy (DPN) is complex, with multiple factors, it shall lead to degeneration of nerve fibers, axonal atrophy,demyelination and nerve cell necrosis finally. The abnormal morphologic function of Schwann cells is considered to be one of the important reasons for the development of DPN, as they are closely related to regeneration of peripheral nerves and their apoptosis, proliferation and differentiation play an important role in the nerve regeneration for axonal regeneration and myelination. According to previous clinical and experimental study of the research group, it is found that Tongluo Tangtai (TLLT) which has the functions of replenishing Qi,activating blood and vein relaxing, can improve the glycolipid metabolism of DPN patients or animal models, reduce relevant inflammatory factors, improve neurologic symptoms and nerve conduction velocity, inhibit the Schwann cell apoptosis of sciatic nerves on rats, decrease the Casepase-3 protein kinase and increase the expression of IGF-1 mRNA. And, the apoptosis, proliferation and differentiation of Schwann cells are closely related to two signal transduction pathways of key cells, namely PEAK-CHOP-Gaspase-12 and Wnt/β-catenin. We think that Tongluo Tangtai may take effect by means of regulating such signals, thus reducing the DPN damages and promoting the regeneration of nerves. Therefore,based on culturing cells in vivo and in vitro, we plan to study that if TLLT can take effect by acting on the apoptosis and (or) proliferation signal pathways of Schwann cells so as to confirm the possible target spots, with electrophysiological detection, histological observation of transmission electron microscopy, RT-PCR, immunofluorescent histochemical method, ELISA, MTT and flow cytometry method and etc. In this case, we can explore the essence of science for DPN treatment with such therapy from a new perspective, thus developing new ideasand methods for DPN treatment with traditional Chinese medicine.
