先天性巨结肠症(HSCR)是小儿先天性消化道常见畸形,因其发病机制不明确,导致诊疗困难。我们前期研究发现纤维粘连蛋白与Neuroligins基因间存有密切地相互调控,证实HSCR的发病机制是“基因异常”和“肠壁微环境”共同作用的结果。预实验发现对肠神经系统突触发生和成熟起重要作用的Neuroligins基因与TIMP3、Sonic hedgehog/Gli(Shh/Gli)通路密切相关。基于此,提出科学假设:“TIMP3可能通过激活Shh/Gli通路,抑制Neuroligins的表达,进而调控肠神经嵴细胞的迁移和定植,从而参与HSCR的发生发展”。本课题利用免疫组化、PCR、干细胞分离培养、病毒转染等分子生物学技术,分别对动物模型、人体标本、细胞进行试验,探讨TIMP3通过Shh/Gli通路调节Nlgns基因诱发HSCR发病机制,为临床基因治疗、胎前干预提供潜在的靶点,探索新的诊疗方法。

Hirschsprung's disease (HSCR) is a common malformation of congenital gastrointestinal tract in children, whose unclear pathogenesis can lead to difficult diagnosis, treatment and poor prognosis. Our previous research found that there is a close mutual regulation between the extracellular matrix fibronectin and Neuroligins (Nlgns) genes, confirming that the pathogenesis of HSCR is the result of the joint action of "gene abnormalities" and " microenvironment changes". Pre-experiments found that the Nlgns gene, which plays an important role in synaptic development, maturation, and function of the enteric nervous system, is closely related to the TIMP3 and Sonic hedgehog / Gli (Shh / Gli) pathways, while the Shh/Gli pathway may be an important signaling pathway connecting extracellular matrix with Nlgns gene. Based on this, a scientific hypothesis is proposed: "TIMP3 can activate the Shh / Gli pathway to cause the down-regulation of the target gene Nlgns, regulate the migration and proliferation of gut neural crest cells, and cause the loss of distal ganglion cells and HSCR." In this study, molecular biological techniques such as immunohistochemistry, immunofluorescence staining, PCR, stem cell isolation, gene transfection and cell apoptosis will be used to conduct experiments on animal models, human specimens and cells at the protein and gene levels, respectively. The Shh / Gli pathway regulates the pathogenesis of HSCR induced by the Nlgn genes, providing potential targets for clinical gene therapy and prenatal intervention, and explores new therapeutic methods.
先天性巨结肠症(HSCR)是小儿先天性消化道常见畸形,因其发病机制不明确,干细胞治疗方案也不完善,导致诊疗方面存在困难。.在发病研究方面,通过WB、QRT-PCR及免疫组化等手段检测HSCR患儿结肠组织MMP9/WNT/β-catenin表达水平,细胞实验检测MMP9-siRNA、β-Catenin-siRNA处理后相关基因表达情况,动物实验检测MMP9抑制剂及WNT/β-catenin抑制剂处理后相关基因的表达水平。在镁离子治疗方面,采用WB、PCR、IF及多电极阵列技术(multielectrode array techniques)评估镁离子对ENCCs分化的影响;利用表面等离子共振(SPR) 检测镁离子与关键糖酵解基因的结合亲和力。将水凝胶ENCCs嵌合体移植到实验动物的皮下和肾囊中,评估体内镁离子对ENCC分化的影响。.在发病研究方面,我们发现越靠近病变结肠段,MMP9与Wnt/β-Catenin的基因和蛋白水平表达水平逐渐下降;在胚胎鼠结肠组织使用MMP9 抑制剂和 Wnt/β-Catenin 抑制剂孵育后,MMP9 或 Wnt/β-Catenin 的表达水平下降。PC12细胞分别转染MMP9-siRNA、β-Catenin-siRNA后,结果提示均提示MMP9、Wnt1、β-Catenin 的表达水平下降,证明三者相互影响;在镁离子治疗方面,我们通过实验证明镁离子能够促进ENCC糖酵解,同时对PFKFB3进行过表达时发现糖酵解相关基因mRNA表达水平升高,采用表面等离子体共振进一步验证镁离子与PFKFB3有很强的结合关系;将的ENCCs移植到实验动物体内,结果提示在体内高镁环境中,ENCCs的突触数量增加,促进了ENCCs在体内向神经元的分化。.由此可以得出MMP9与Wnt/β-Catenin 信号通路之间互相调控,影响HSCR的发生;同时镁离子通过靶向PFKFB3调节ENCCs的糖酵解途径并有促进ENCCs在体内向神经元分化的潜质。
