胃肠道间质瘤(Gastrointestinal stromal tumor,GIST)是胃肠道最常见的间叶源性肿瘤。其基因突变类型对明确诊断、评估生物学行为及指导治疗极为重要。C-kit基因11号外显子突变GIST患者接受伊马替尼治疗疗效最好。C-kit基因11号外显子突变中涉及密码子557/558缺失突变者,肿瘤预后差。临床上部分GIST患者需接受活检进行基因学检测。作为有创检查,活检存在一定风险。能谱CT成像是能定量反映病变血供情况及物质代谢变化的成像技术。我们前期研究发现不同基因突变类型GIST CT表现(含强化程度)存在一定差异。以此为基础,本课题创新性纳入GIST患者行能谱CT增强扫描,以基因突变类型为观察指标,探索其能谱CT表现差异,并进一步揭示其分子病理学改变。本项目有望为无创评估GIST基因突变类型提供依据,进一步指导个性化治疗方案的制定,达到精准治疗的目的。
英文摘要
Gastrointestinal stromal tumor (GIST) is the most common mesenchymal tumors of gastrointestinal tract. The gene mutation type of GIST is of great importance for diagnosis, assessment of the biological behavior and the treatment. GIST with C-kit exon 11 mutation has the best treatment response to imatinib. And these with C-kit exon 11 mutation involving 557/558 codon deletion have poor prognosis. In clinical, a part of patients undergo biopsy for gene mutation examination. As an invasive examination, the biopsy often has certain complications. Energy spectrum CT imaging can quantitatively reflect the changes of blood supply and substance metabolism. Our previous studies have found that there are some differences in CT image features (including enhancement degree) of GIST with different gene mutation types. Based on this, our study is designed innovatively to include GIST patients for contrast-enhanced energy spectrum CT scanning, to compare the energy spectrum CT features and parameters according the gene mutation type. We will further reveal the mechanism of molecular pathological changes. This project is expected to provide a basis for the preoperative non-invasive evaluation of gene mutation types in GIST, and further guide the formulation of personalized treatment plans to achieve the goal of precise treatment.
