基于前期对宁夏包虫病高流行区监测调查发现:在国家包虫病控制项目(家犬驱虫)实施期间,家犬感染率居高不降;还发现当地少儿(6-15岁)人群血抗包虫抗体水平有普遍升高趋势(近期误食虫卵)。这些预警信息,激发课题组拟查国家控制项目在这地区落实效果不力的原因。选择被预警区(西吉县)的4个自然村,用因子随机对照的综合干预矩阵(犬驱虫/不驱虫,人教育/不教育)做试验(不同年份调换每村的干预内容)。并每年检查4个村的家犬粪(棘球绦虫DNA/卵),少儿血抗包虫特异抗体(感染率),及少年/犬主人有关包虫病知识(问卷)掌握情况。比较上述在不同干预试验条件下各个指标的检查结果,分析找出国家包虫病控制项目实施模式下,发生包虫病传播预警信息的原因,指导克服本地控制项目效果不力的方案出台。该项目成果将对改善当地人群健康有重要意义。建立实时监测反馈综合干预策略必将为可持续控制方案和全球消除包虫病战略做出贡献。

Based on the previous investigation in the high-endemic area of echinococcosis in Ningxia Autonomous Region (NHAR), the infection rate of domestic dogs was high during the implementation of the national echinococcosis control project (owner dog deworming); The infections among local children / adolescents (6-15 years old) were also found. There is a general increase of the anti-Echinococcus spp. antibody levels in blood in the young population ( indicating recently ingested Echinococcus eggs). These warning messages regarding Echinococcus spp. transmission have motivated the research team to investigate the reasons for the ineffective implementation of national control projects in this area. Four natural villages with the early warning information in the area of the epidemic county (Xiji County) were selected, and intervention trials were conducted using a comprehensive intervention matrix program of randomized controlled trials (dog deworming/non-deworming; human education/non-education) (interchanges in each village were implemented the intervention trials in different years) . Each year, the dog copro-DNA test (Echinococcus spp. DNA/egg) in four villages, the anti-Echinococcus spp. specific antibodies (infection rate) in children/adolescents, and the echinococcosis knowledge (questionnaire) of adolescents/dog owners are examined. Comparing and analyzing the above-mentioned results of various factors under different intervention test conditions, will reveal the reasons for the occurrence of the early warning information on Echinococcus spp. transmission during the implementation of the national Echinococcus control project, and guide the introduction of plans to overcome local control defects. The outcome of the project will be important to improve the health of the local population; the identified integrated intervention strategy will certainly contribute to sustainable control programmes and global strategies to eliminate echinococcosis.
