帕金森病(Parkinson's disease,PD)认知障碍是非运动损伤中最常见的致残症状之一,明确其进展机制及寻找可靠的标志物对临床管理意义重大。研究证实,PD认知障碍患者海马并非同质性的损伤,不同亚区间存在差异,因此,将其作为一个整体研究并不能阐明其作用,且海马亚区-腹侧纹状体-前额叶环路损伤模式仍不清楚。申请者前期发现了该环路损伤的一些特征:海马角1区是早期萎缩亚区;随着PD认知损伤进展,海马角与前额叶及壳核功能连接从减低到增高;轻度认知障碍患者腹侧纹状体多巴胺转运体水平增高且与认知相关。因此,我们假设PD认知障碍演变中海马亚区-腹侧纹状体-前额叶环路损伤存在差异,多巴胺能在早期就参与认知调节,海马角1区损伤可能是疾病进展的标志物。本项目将联合多模态PET/MRI,从分子、功能及结构水平研究上述环路的损伤模式及多巴胺能对其调节作用,以期为临床提供PD认知损伤进展的客观证据。

Cognitive impairment is one of the most common disabling symptom in non-motor features of Parkinson's disease (PD). The core of clinical management depends on elucidating the progress mechanism and objective assessment. Although hippocampus has been proved to be involved in the progression of cognitive impairment of PD, its subfields are heterogeneously damaged, which means considering the hippocampus as a solitude entity may not adequately elucidate its function in PD. Moreover, the injury patterns and regulatory mechanisms of different hippocampal subfields-ventral striatum-prefrontal loop remains equivocal. Our previous research found several features in the hippocampal subfields-ventral striatum-prefrontal loop. Firstly, we found that the alteration of grey matter volume differed in hippocampal subfields in patients with cognitive impairment, with cornu ammonis-1 reduction in the early stage; secondly, our results showed that functional connectivity between the cornu ammonis and prefrontal cortex and putamen increased in patients with cognitive dysfunction, while functional connectivity between cornu ammonis and prefrontal cortex decreased in patients without cognitive impairment. Thirdly, the dopamine transporter of the ventral striatum was significantly increased in patients with mild cognitive impairment of PD, as well as associated with cognitive rating scores. Thus, we hypothesized that injury of the different hippocampal subfields-ventral striatum-prefrontal loop may vary in patients with cognitive impairment, which would be influenced by dopaminergic alteration in the early stage. The cornu ammonis-1 may be an early vulnerable subregion and a biomarker of progression of the disease. Taking advantage of multi-modal PET/MRI techniques, we will explore the functional and structural changes of the different hippocampal subfields-ventral striatum-prefrontal loop and the effect of dopaminergic alteration, which could revealed the injury patterns as well as regulatory mechanisms of the hippocampus, hoping to provide objective proof of progression for clinical management.
帕金森病(PD)是常见的中枢神经系统退行性病变,其认知功能障碍是非运动症状中最常见的症状之一,探讨其认知损伤机制及寻找可靠的影像学指标具有重要临床价值。本项目进行了如下探索:1.采集PD患者及正常对照临床一般资料、运动及认知评分及影像学数据,基于3D-T1结构像分割海马亚区体积,获得认知损伤进展过程中海马亚区萎缩特征;2.基于第一步得到的海马亚区损伤区域,结合rs-fMRI分析海马亚区及全脑功能损伤模式,探讨PD认知损伤进展机制;3.基于PET数据分析PD疾病早期及中晚期脑代谢相对异常脑区及脑网络拓扑属性关键节点改变模式,分析早、中晚期PD的脑部病变范围、影像特征;4.基于全脑体积分割,结合PET研究PD患者及帕金森综合征患者脑内脉络丛体积的变化特征,探索鉴别诊断影像学标记物。研究结果表明:1.PD认知损伤早期,海马亚区特异性损伤亚区位于为苔藓纤维通路;2.与PD无认知损伤组相比,PD认知损伤组双侧齿状回颗粒层联合海马角4区与后部默认网络及右侧视觉皮层(v1区)功能连接增高,且在PD轻度认知损伤组中,该功能连接增高与认知评分负相关,说明海马齿状回颗粒层联合海马角4区与后部默认网络功能改变是PD认知功能损伤进展的重要脑区;3.与早期PD组比较,中晚期PD组屏状核、中脑、左侧辅助运动区等异常代谢脑区,且脑桥、小脑、顶枕叶多个代谢异常脑区范围更大。网络拓扑属性结果显示与早期PD相比,中晚期PD组边缘系统部分Hub节点综合排序减弱,额、枕叶部分Hub节点综合排序增强;4.进行性核上性麻痹患者脉络丛体积较PD患者显著增大,两患者组鉴别曲线下面积为0.84,且脉络丛体积与黑质、蓝斑、壳核及丘脑的平均摄取值正相关;在PSP与PD患者中,脉络丛体积与运动评分正相关,与认知评分负相关,该结果表明脉络丛体积改变是鉴别进行性核上性麻痹及PD的可靠影像学指标。本研究结果为进一步理解PD认知损伤提供了新思路,为临床鉴别诊断提供了可靠影像学指标。
