高血压是慢性肾病发生发展的最重要原因之一,高血压肾病进展到终末期肾病的主要因素之一是肾间质纤维化。G蛋白偶联受体(GPCR)作为人类疾病治疗最重要的靶点,一直受到广泛关注。GPR97是一个新发现的粘附性GPCR家族的成员,其在肾纤维化中的作用尚不清楚。我们预实验中首次证实GPR97在高血压条件下的在肾小管上皮细胞中特异表达增加并与Hippo/Yap信号通路相关,明确GPR97参与了高血压肾病的发生发展。为此,本课题以GPR97为中心,运用cre-loxp条件基因敲除技术构建特异性肾小管上皮细胞GPR97基因敲除小鼠等手段深入探讨GPR97在高血压肾病中的作用及探讨GPR97-Hippo/Yap在肾小管上皮细胞中的分子调控机制;并进一步以慢病毒敲减GPR97作为高血压肾病治疗手段的初步探索。此研究将拓展对GPR97生物学功能的新认识,为高血压肾病的防治提供新靶点和思路,具有重要的理论价值。

Hypertension is one of the most important causes in the pathogenesis of chronic kidney disease. Renal tubulointerstitial fibrosis plays a crucial role in progression of hypertensive nephropathy to end-stage renal disease. G protein-coupled receptors (GPCR) are major targets of pharmaceutical drugs. The adhesion GPCRs that are a recently defined subfamily of GPCRs take center stage in the study of human diseases. GPR97, a newly identified adhesion GPCR, is demonstrated a modulator in immune system. However, it is unknown whether GPR97 contributes to the regulation of renal functions and the role of GPR97 in hypertensive nephropathy remains unclear. Our preliminary studies showed that GPR97 was up-regulated in the kidney from mice suffered hypertensive nephropathy. We also found that GPR97 was characteristically up-regulated in renal tubular epithelial cells and involved in modulating Hippo/Yap signaling pathway. Hippo/Yap signaling pathway play an important role in the pathogenesis of renal tubulointerstitial fibrosis and was associated with regulation of inflammation and renal tubular epithelial-to-mesenchymal transition (EMT). These finding indicate that GPR97 participates in the occurrence and development of hypertensive nephropathy. Therefore, the present study is designed to explore the regulatory effect of GPR97 in hypertensive nephropathy and investigate GPR97-Hippo/Yap signaling pathway in renal tubular epithelium both in vitro and in vivo animal studies using renal tubular epithelium-specific deletion of GPR97 mice. We then evaluated the possibility of down-regulating GPR97 by lentivirus as a therapeutic drug for the treatment of hypertensive nephropathy. In this study, we postulate that GPR97 is a major modulator of Hippo/Yap signaling pathway in renal inflammation and EMT, representing a putative target to ameliorate human renal tubulointerstitial fibrosis.
恶性高血压能够迅速导致不可逆的肾衰,肾间质纤维化是高血压肾病进展到终末期肾病的主要原因之一,目前尚无有效的方法治疗肾纤维化,肾纤维化的病人最终会进展到终末期肾病。GPCR是一大类通过G蛋白介导其生物效应的膜受体的总称,是潜在的药物靶点,GPR97是一种粘附性GPCR,目前国内外对GPR97的研究较少,主要集中于对免疫系统调节,如过敏性哮喘、肥胖中的炎症反应和B淋巴细胞的调控等,本课题组前期研究发现,GPR97能够促进急性肾损伤的发展,但GPR97在肾脏疾病特别是高血压肾病中肾纤维化的形成和发展过程中的作用,迄今未见报道。在本课题中,我们发现GPR97 在人的肾活检组织中的肾小管中表达明显升高,并通过对高血压肾损伤模型的检测,发现GPR97在DOCA-盐敏感性高血压模型肾小管中明显升高,推测GPR97可能在高血压肾损伤的发生发展中发挥重要作用。我们应用GPR97敲除小鼠对GPR97在高血压肾损伤中的作用进行了探索,发现在敲除GPR97之后,明显改善了DOCA-盐敏感性高血压模型小鼠的肾功能,减少了高血压引起肾小球损伤和肾间质纤维化,并进一步检测到纤维化相关蛋白及上皮-间质细胞转分化(EMT)相关蛋白的表达受到抑制。转换生长因子β(TGF-β)/smad2信号通路是间质纤维化的重要调控因素,我们发现,敲除GPR97能够明显抑制TGF-β/smad2信号通路的转导,并且抑制其调控因子Yap的表达。TGF-β/smad2信号通路的转导收到內吞途径的影响,clathrin介导的內吞,促进其信号转导,而caveolin介导的內吞途径则促进TβRII的降解,抑制其信号转导。本研究发现GPR97能够调控TGF-β/smad2信号通路的內吞过程,敲减GPR97能够抑制TGF-β二型受体 (TβRII) clathrin介导的內吞过程,从而抑制TGF-β/smad2信号通路。本研究进一步证实,RhoA在TβRII的內吞过程中发挥重要作用,敲除GPR97能够明显抑制TGF-β诱导的RhoA激活,由此我们认为,GPR97通过激活RhoA促进TβRII的clathrin介导的內吞过程,促进TGF-β/smad2信号通路的信号转导,从而促进高血压引起的肾损伤。本课题完善高血压肾纤维化发病的分子生物学机制,并对疾病的防治筛选新的有效靶点,具有重要的理论意义和潜在的应用价值。
