Fibroblasts(Fb) are the main effector cells of hypertrophic scars. The discovery of more precise targeted therapies may be facilitated by the investigation of critical cell subtype and its related pathogenesis. The preliminary research of our group show that the fraction of CD39+ Fb is increased in hypertrophic scars and is positively correlated with the scar severity; the expression of IL-11 is increased significantly in CD39+ Fb according to the gene expression trend analysis; and the inhibition of CD39 could effectively downregulate the expression of IL-11 and impede the formation of hypertrophic scar. Recent researches showed that the activation of specific adenosine receptors in CD39 downstream signaling pathway could promote the secretion of IL-11, leading to exacerbation of tissue fibrosis. Based on these findings, we propose that CD39+ Fb may promote the secretion of IL-11 by activating those specific adenosine receptors and their downstream signaling pathways, which ultimately leads to the formation of hypertrophic scars. In this study, we intend to explore the role of CD39+ Fb on IL-11 secretion, myofibroblast activation and hypertrophic scar development via the gene overexpression and knockdown cell lines and the transgenic mice model. Further, the main types of adenosine receptor responsible for the regulation of IL-11 expression will be screened and their mechanisms to facilitate IL-11 expression along with the possible therapeutic targets will be investigated by means of adenosine receptor agonists, PCR array and siRNA interference. This project is expected to uncover the mechanism of hypertrophic scar formation from a new perspective of cell subtypes, which may contribute to the development of new targeted therapies.
增生性瘢痕是一种常见的皮肤疾病,其致病机制尚未完全明确。成纤维细胞(Fb)是增生性瘢痕的主要效应细胞,瘢痕增生通常表现为成纤维细胞功能的异常激活和细胞外基质成分的过度沉积。本研究发现,在增生性瘢痕的临床样本和动物模型中,CD39+成纤维细胞(CD39+Fb)的数量显著增加,其数量与临床瘢痕增生的严重程度呈正相关。通过细胞分选和转录组学分析,发现正常皮肤中的CD39+Fb在向增生性瘢痕转化的过程中,TGF-β通路被激活,促使其向间质细胞表型转化,并显著提高细胞的氧化磷酸化水平和增殖能力。在增生性瘢痕和皮肤纤维化模型中,使用CD39的小分子抑制剂POM-1能够有效抑制瘢痕形成和皮肤纤维化,减少胶原沉积和成纤维细胞激活。此外,我们构建了CD39成纤维细胞特异性过表达(CD39-Tg)和敲除(CD39-eCKO)小鼠模型。结果表明,CD39在成纤维细胞中的敲除会导致皮肤愈合速率减慢,并减轻瘢痕形成。. 转录组测序分析发现CD39+Fb分泌较高水平的促纤维化因子IL-11,并且TGF-β能够维持CD39+Fb的细胞表型并促进IL-11的分泌。通过组织免疫组化染色,我们证实了IL-11在增生性瘢痕的临床样本中表达上调。在体外实验中,IL-11的刺激能够促进成纤维细胞的基质分泌并诱导其转化为肌成纤维细胞。利用siRNA阻断IL-11通路能够逆转成纤维细胞的功能激活。体内敲除IL-11能够抑制机械力诱导的增生性瘢痕形成。局部或全身给药IL-11治疗性抗体均能显著减少瘢痕组织的重量,促进瘢痕组织萎缩、基质降解,并减轻纤维化。进一步机制研究发现,CD39+Fb的IL-11分泌能力明显高于CD39-Fb,同时CD39-Fb对IL-11的敏感性高于CD39+Fb。通过使用小分子抑制剂POM-1和siRNA技术,我们证实敲低CD39能够显著降低IL-11的表达并抑制成纤维细胞向肌成纤维细胞的转化。. 综上所述,本研究揭示了CD39+Fb通过分泌IL-11,诱导成纤维细胞向肌成纤维细胞转化及其细胞外基质分泌功能的激活,从而促进瘢痕增生。这一发现为靶向CD39和IL-11通路的治疗提供了新的思路。其中,IL-11治疗性抗体已完成I期临床安全性试验,结果显示药物安全性良好,后续将推进该药物在瘢痕患者中的II期临床试验,为临床提供新策略。
