儿童近视的发生发展机制尚不清楚,防治措施有限。我们近期发现增加儿童户外时间和睡眠时间均可使眼轴长度更短、近视度数更低;持续光照发育期大鼠可使包含黑视素的光感受性视网膜神经节细胞(ipRGCs)数量增加,且维持小鼠正常昼夜节律需要功能完整的ipRGCs。据此提出假说:光照和昼夜节律失调可刺激ipRGCs及黑视素表达,局部影响视网膜生物钟,上行作用于视交叉上核(SCN)并反馈于视网膜,调控屈光发育过程。本项目将探讨不同强度光照对发育期小鼠ipRGCs及黑视素、昼夜节律基因、多巴胺和褪黑素的影响,明确ipRGCs在光照影响近视过程中的作用和机制;建立小鼠形觉剥夺性近视模型,探讨ipRGCs在昼夜节律失调下对近视的影响和机制;阻断ipRGCs不同亚型、上调黑视素,探讨SCN节律功能及视网膜生物钟的变化,以期阐明ipRGCs在日节律光协同化影响屈光发育过程中的作用和机制,为近视防治寻找新的理论依据。
英文摘要
The mechanism of myopia in children is not clear, and the prevention and treatment measures are limited. We recently found that increased outdoor time and sleep time in children resulted in shorter axial length and lower myopia. Continuous illumination increased the number of intrinsically photosensitive retinal ganglion cells (ipRGCs) containing melanopsin in developing rats, and fully functional ipRGCs were required to maintain normal circadian rhythms in mice. It is therefore hypothesized that light and circadian rhythm dysregulation can stimulate the expression of ipRGCs and melanopsin, locally affecting retinal biological clock, upstream action on the suprachiasmatic nucleus (SCN) and feed back to the retina to regulate the process of refractive development. In this project, the effects of different intensities of light on ipRGCs, melanopsin, circadian rhythm genes, dopamine and melatonin in developing mice will be firstly investigated to clarify the role and mechanism of ipRGCs in the process of light affecting myopia. A mouse model of form-deprivation myopia will be established to investigate the effect and mechanism of ipRGCs on myopia under circadian rhythm disorder. Blocking different subtypes of ipRGCs, upregulating melanopsin, and exploring the changes of SCN rhythm function and retinal biological clock, in order to elucidate the role and mechanism of ipRGCs in the process of diurnal rhythm light synergism affecting the development of refraction, and to find a new theoretical basis for the prevention and treatment of myopia.
近视是影响儿童身心健康的重大公共卫生问题。本团队前期发现户外时间和睡眠时间均与儿童的近视发生发展有关,提示昼夜节律紊乱可能是导致近视的重要因素,光照和昼夜节律失调可刺激包含黑视素的光感受性视网膜神经节细胞(ipRGCs)表达。本课题组在此基础上,进一步对比观察不同光照强度(10 lux、300 lux、3000 lux)和不同光/暗周期(6L/18D、12L/12D、6L/18D、持续光照)下,自然屈光发育和单眼形觉剥夺性近视(FDM)小鼠眼生物学参数、视网膜电生理功能及分子生物学指标含量的变化,探讨昼夜节律失调影响小鼠形觉剥夺性近视过程中的作用和机制;进而上/下调 melanopsin对C57BL/6小鼠中枢SCN节律功能及局部视网膜生物节律基因、多巴胺的影响,探索ipRGCs在日节律光协同化影响屈光发育过程中的作用和机制。本研究发现,在正常昼夜节律(12L/12D)下,3000 lux光照可抑制小鼠FDM,减少视网膜Bmal1、Clock、Cry1、Per3表达;并促使小鼠自然屈光发育向远视发展,减少视网膜melanopsin含量。当光暗周期紊乱(18L/6D、持续光照)时,300 lux和3000 lux光照均可导致小鼠自然屈光发育向近视漂,显著抑制编码ipRGCs的基因Opn4表达,增加Per3、Cry1的表达,但多巴胺含量无显著性差异;在光暗周期紊乱情况下,高强度持续光照仍可抑制小鼠FDM,小鼠视网膜Opn4、Per3、Cry1含量增加,但多巴胺含量无显著性差异。进一步研究发现,下调视网膜melanopsin使小鼠自然屈光发育向近视漂移,视网膜内Bmal1、Clock、Per3、Cry1含量下降,上调视网膜melanopsin使小鼠向远视漂移、视网膜Cry1含量上升,但上/下调melanopsin均未导致小鼠生物节律及视网膜多巴胺含量发生显著性变化。因此,本项目证实ipRGCs 及 melanopsin 参与了日节律光协同化影响屈光发育过程,该过程主要通过调控视网膜内昼夜节律基因实现,未改变SCN中枢节律及视网膜多巴胺表达。本研究结果为调整光照和昼夜节律进行近视干预提供科学理论依据,应关注儿童所处环境的光照强度和光 / 暗周期的合理性,避免长时间处于光暗周期紊乱(如 18L/6D、持续光照)的环境中,以降低近视发生的风险,为近视防控领域提供全新的干预策略。
