The local inflammatory lesions in rheumatoid joints display characteristics that are shared with several chronic inflammatory reactions that involve local Tlymphocyte activation. Thus, whereas normal synovial tissue only contains a few class I1 expressing cells capable of antigen-presentation to T lymphocytes (1-5), the rheumatoid synovial tissue contains a large number of various class I1 expressing cells, some of which have a high antigen-presenting capacity (1-5). Evidence that a local T lymphocyte activation in RA occurs in contact with these synovial class I1 expressing cells stems from the demonstration that synovial T lymphocytes express activation markers to a greater extent than do peripheral T lymphocytes from the same rheumatoid patients (1, 6, 7). ActivatedTlymphocytes such as those present in the RA synovium may secrete gamma-interferon and possibly other molecules with capacity to induce class I1 antigen expression in previously class I1 negative cells (8, 9). It was therefore concluded that conditions are present in the rheumatoid joint for a self-perpetuating class I1 dependent T lymphocyte activation (1, 3, 4) of a kind that has subsequently been suggested to occur also in a number of other tissue-specific inflammatory lesions with a suspected autoimmune background (10-14).
