The osmotic component results from, first, a decreased synthesis of the brush border disaccharidase enzymes (lactase, sucrase and maltase), and subsequently death of the villous enterocytes leading to flattening of the villi. The diarrhoea in well-nourished children persists for an average of 6 days but in some may persist much longer. There is no evidence to suggest that milk and solids should be withheld, despite the loss of dissaccharidase activity. It is probable that we are all infected with rotavirus at regular intervals throughout our lifetimes but that it is only on the first few occasions that infection results in disease, unless an entirely new rotavirus strain intervenes', A cohort study in Mexico has demonstrated that after one rotavirus infection a child has a 62% chance of developing a subsequent infection and 23% chance of developing diarrhoea, after two and three attacks the chances were 40% and 17%, and 34% and 8%, respectively', After many years of development a live simian reassortant rotavirus vaccine has been produced which has been shown to be as effective in developing as developed countries5,1O, l l .
