Dear editor, The recent advance of optical coherence tomography (OCT) technology enables the measurement of choroidal thickness precisely in vivo and obtains more information on the role of the choroid in various systemic diseases such as diabetic mellitus, systemic hypertension, and central nervous degenerative diseases. Obstructive sleep apnea syndrome (OSAS) and type 2 diabetic mellitus (T2DM) share a pathophysiologic pathway, specifically insulin resistance. The blood glucose in patients with strong insulin resistance usually is not well controlled, which leads to microvascular and macrovascular complications. Many studies have assessed the relationship between OSAS and T2DM. OSAS is prevalent in patients hospitalized for poorly controlled T2DM, with 34% for mild OSAS, 19% for moderate OSAS, and 10% for severe OSAS.[1] A meta-analysis showed that OSAS was related to higher prevalence of T2DM. The pooled relative risks of T2DM were 1.22 (95% CI 0.91–1.63) for mild OSAS and 1.63 (95% CI 1.09 to 2.45) for moderate-severe OSAS compared to those without OSAS.Although some studies did not observe correlations between OSAS and diabetic retinopathy (DR), various recent studies confirmed that OSAS was an independent risk factor for presence and exacerbation of DR and represented a novel biomarker of DR [2, 3]. OSAS has also been correlated with poor outcomes and more bevacizumab injections in patients with diabetic macular edema compared to age-matched controls [4].
