In the two decades since Berger’s original description, insight into the immunopathogenesis of IgA-related nephropathies has provided our most direct evidence of induction of nephritis by infection, and has improved our understanding of the diverse interactions of the immune system with the environment and kidney. These nephropathies have become the most frequently detected renal diseases in developed countries1. Variations in policy towards renal biopsy are likely to account for the lower reported incidences in the UK and possibly in the USA2–4. Once considered benign, it is now recognized that inexorable progression of disease often occurs in the largest subgroup, those with low-grade urinary abnormalities2,5, and that the IgA nephropathies are among the commonest causes of end-stage renal failure6.
