Clinical application of technology for human in vitro fertilization (IVF) and embryo transfer in the treatment of various types of male and female infertility is now widespread. Although the methods for human IVF appear to be well established, the application of this technique has proceeded without a sound foundation of information at the level of cell biology. Inadequate information exists on the relationship of fertilization dysfunction to human infertility. There are few clinical tests for assessing the competence of human spermatozoa to fertilize the human oocyte. In current practice, a trial and error approach must be taken in which the fertilizing competence of sperm and oocyte are tested experimentally, with subsequent transfer of the embryo if apparently normal fertilization occurs. Significant progress in the recognition and therapy of fertilization dysfunction in the male or female will require additions to our knowledge of human fertilization biology and pathology. The cell biology of mammalian fertilization is an area of considerable contemporary investigation. In particular, the experimental evidence supporting certain models of sperm-zona pellucida interaction, developed with laboratory animals, is strong enough to warrant testing the models directly with human gametes. In our laboratory we have used in vitro systems to study the biology of human sperm-zona pellucida interaction at the cellular level. This knowledge will be used in the investigation of abnormalities of sperm function that could lead to fertilization failure. aThe experiments described in this chapter were supported by National Institutes of Health Grants HD15149 and HD19587.
